PATIENTS NOT ONLY RECEIVE FREE MEDICATIONS BUT ALSO EXPERIENCE POSITIVE HEALTH OUTCOMES THROUGH THE PHARMACY'S CLINICAL PROGRAM. CLINICAL SERVICES INCLUDE DIABETES SELF-MANAGEMENT EDUCATION, TOBACCO CESSATION, MEDICATION THERAPY MANAGEMENT, AND HEALTH SCREENINGS. THESE INTERVENTIONS DECREASE EMERGENCY ROOM VISITS AND HOSPITALIZATIONS, IMPROVE MEDICATION ADHERENCE, AND IMPROVE DIABETES CONTROL.
