“A draft copy of the federal form 990 is provided to the treasurer and finance and audit committee for review and comment and then to the board of directors for review and comment in order to approve the draft form 990 prior to filing with the internal revenue service.”

“The foundation has a conflict of interest policy in place, and board members are asked to bring any potential conflicts to the attention of the president and to the board of directors. Conflicts of interest are monitored by the board of directors informally. Before a director is added to the board of directors, the full board of directors discusses the director nominee at a board meeting, reviews their background and experience and discusses any board member connections with or potential conflicts with each proposed director before any vote to add the proposed director to the board of directors. Any board member with a conflict of interest is asked to bring this conflict to the attention of the president and the board of directors and is asked to recuse herself or himself from any discussion or vote on any item related to said conflict. The foundation is developing a conflict of interest questionnaire for each board member to complete annually.”
