Form 990, Part VI, Line 4: Description of Significant Changes to Organizational Documents

Proposed changes in By-laws to comprehensively outline (a) staggered terms of board members and (b) job descriptions for elected officers.Changes were unanimously approved with clarifications regarding the definition of books and records.To become compliant with the amended By-laws, staggered terms were established for all board members.

Form 990, Part VI, Line 11B: Form 990 Review Process

The Chief Operations Officer will submit the draft 990 to the Board chair as well as to the Finance Committee of the Board of Directors to review prior to final submission.

Form 990, Part VI, Line 12C: Explanation of Monitoring and Enforcement of Conflicts

Annual Statements. Each director, principal officer, and member of a committee with governing board delegated powers shall annually sign a statement which affirms such person:(1) has received a copy of the conflicts of interest policy,(2) has read and understands the policy,(3) has agreed to comply with the policy, and(4) understands the corporation is charitable and in order to maintain its federal tax exemption it must engage primarily in activities which accomplish one or more of its tax-exempt purposes.If any conflicts are noted, the Board Chair requests the party recuse themselves from voting on any action that directly impacts the affected relationship.Periodic Reviews. To ensure the corporation operates in a manner consistent with charitable purposes and does not engage in activities that could jeopardize its tax-exempt status, periodic reviews shall be conducted. The periodic reviews shall, at a minimum, include the following subjects:(1) Whether compensation arrangements and benefits are reasonable, based on competent survey information, and the result of arms-length bargaining.(2) Whether partnerships, joint ventures, and arrangements with management organizations conform to the corporation's written policies, are properly recorded, reflect reasonable investment or payments for goods and services, further charitable purposes, and do not result in inurement, impermissible private benefit, or in an excess benefit transaction.Use of Outside Experts. When conducting the periodic reviews the corporation may, but need not, use outside advisors. If outside experts are used, their use shall not relieve the governing board of its responsibility for ensuring periodic reviews are conducted.

IRS990/Desc0GymThe PWR!Gym is a model community wellness center where exercise is medicine for those living with Parkinson Disease. The gym averages 115 members per month. Each member receives a 90 minute physical therapy consultation at the time of enrollment to determine which class is most appropriate for their level of ability. All members are provided a minimum of three days of class per week (each class is one hour in duration). People with Parkinsons who do not live in the Tucson area can participate in exercise intensives at the gym. They work one on one with a physical therapist to address a particular area of functioning (walking, getting up off the floor, etc.). Sessions can take place from 3-5 days for a few hours each day. During this fiscal year, 25 individuals received 230 hours of intensive services from PWR! physical therapists.Gym open house event: January 23rd, 2016 31 members in attendance 1 VolunteerPole walking training series: 4 members trained for a total of 16 hoursWorkshopsPWR! conducts workshops across the nation and the world certifying physical and occupational therapists, as well as fitness instructors, in the PWR! methodology. Therapists learn how to prescribe comprehensive, individualized programming to address multiple symptoms and particular levels of functionality affecting people with Parkinson Disease. Instructors learn how to conduct group and circuit classes for people with Parkinsons, which utilize larger and faster functional movement to reduce symptomology.During this fiscal year, PWR! conducted 25 Workshops and certified 692 professionals. PWR! also held 18 community talks (with 360 people in attendance) about how our programming improves quality of life for people with Parkinson disease. RetreatsPWR! holds a week-long, annual retreat which puts into practice the latest research on exercise and wellness specific to Parkinson Disease. Daily programming includes pole walking, group exercise, educational talks and enrichment activities for people with Parkinsons and their care partners. During this fiscal year, 124 people with PD and care partners attended PWR! retreats. Community EventsBowden Hike: 17 volunteers (not included above) 131 participants.Community outreach and information sessions (presentations and boothing): PWR! staff attended 16 events and had contact with 3,945 individuals
