IN APRIL, RAF PRESENTED THE SOLUTION OF "FORTIFICATION OF IODIZED SALT WITH FOLIC ACID" TO THE ETHIOPIAN GOVERNMENT AT THE TECHNICAL EXPERT CONFERENCE IN ADDIS WHICH WAS HELD IN CONJUNCTION WITH THE CLINICAL SERVICES DIRECTORATE OF THE MINISTRY OF HEALTH. THE MINISTRY OF HEALTH ACCEPTED THE SUBSEQUENT POLICY BRIEF MADE BY THE ETHIOPIAN PUBLIC HEALTH INSTITUTE ON MAY 15. THIS WAS IMMEDIATELY FOLLOWED BY A DIRECTIVE TO ALLOW VOLUNTARY FOLIC ACID FORTIFICATION OF WHEAT FLOUR IN ETHIOPIA. A GOOD SUMMATION OF THE CONFERENCE RESULTS IS THE EDITORIAL BY DR. AFEWORK OF TIGRAY PUBLISHED IN THE PROMINENT NE AFRICAN HEALTH SCIENCES JOURNAL ON APRIL23, IMMEDIATELY AFTER THE CONFERENCE: EAST AFR.J.HEALTH SCI.1(10):2019IN SEPTEMBER, RAF ISSUED A GRANT TO THE NUTRITION INTERNATIONAL AND THE ETHIOPIAN PUBLIC HEALTH INSTITUTE TO PROCEED WITH THE PRELIMINARY STUDIES NEEDED TO ASSESS SALT FORTIFICATION WITH FOLIC ACID AND ITS DURABILITY AND PATIENT ACCEPTANCE. RAF ESTIMATES THAT THIS WILL CUT OFF ONE YEAR WAITING BEFORE FUNDING OTHERWISE COULD BE REALIZED, AND THUS SAVE 44.000 PREGNANCIES FROM BEING AFFECTED WITH SBH.IN NOVEMBER, DURING THE SECOND RAF TEAM VISIT, THE PREVENTION TEAM MEMBERS MET WITH REPRESENTATIVES FROM ETHIOPIA'S GATES FOUNDATION OFFICE AND PRESENTED A FUNDING PROPOSAL FOR THE FEEDING STUDY THAT WILL PROVE THAT SALT FORTIFIED WITH FOLIC ACID WILL EFFECTIVELY PREVENT SPINA BIFIDA.THE TEAM ALSO MET WITH THE MINISTRY OF HEALTH AND RECEIVED STRONG ENDORSEMENT FOR THESE STUDIES.IN DECEMBER, THE PRESTIGIOUS JOURNAL SCIENCE PUBLISHED AN ARTICLE: BESET BY NEURAL TUBE DEFECTS, ETHIOPIA MAY FORTIFY SALT. THE GRANT FOR 100 MILLION DOLLARS, IF AWARDED WILL BRING FOOD FORTIFICATION WITH FOLIC ACID TO 17 COUNTRIES WITH THE HIGHEST BIRTH DEFECTS AND SAVE 400.000 LIVES PER YEAR AFTER IMPLEMENTATION.THE GRANT HAS PASSED 3 OUT OF 4 HURDLES AS OF DECEMBER 31,2019.SOCIETY OF ETHIOPIAN NEUROSURGICAL PROFESSIONALS MEETING THE TECHNICAL EXPERT MEETING WAS IMMEDIATELY FOLLOWED BY THE MEETING WHERE RAF GAVE THE KEYNOTE ADDRESS: "AWARENESS, ETHICS AND THE NEW REALITY OF SBH CARE".DURING THE MEETING, THE HYDROCEPHALUS CAMPAIGN WAS OFFICIALLY ENDED AS THE ORIGINAL GOALS WERE EXCEEDED WITH 5000 OPERATIONS AND TRAINING OF 40 NEUROSURGEONS. RAF PRESENTED CERTIFICATES OF APPRECIATION TO DR. HAGOS, AND IN ABSENTIA TO DR. MERSHA FOR THEIR COMMITMENT TO THE CAUSE OF SPINA BIFIDA AND HYDROCEPHALUS IN ETHIOPIA. ALL 5 CENTERS GAVE PRESENTATIONS ABOUT THEIR ACHIEVEMENTS AND OBSTACLES TOWARDS REACHING THE CENTER OF EXCELLENCE GOALS. RAF DISCUSSED BENCHMARKING AND DETAILS OF AFTERCARE.DURING THE FIRST VISIT RAF RECOGNIZED STRUCTURAL LIMITATIONS OF HOSPITALS TO CREATE COHERENT LONGSTANDING AFTERCARE TEAMS. ONE OF THE PRIMARY BOTTLENECKS IS THE FREQUENT TURNOVER OF NURSING AND MEDICAL STAFF. MR. THE DATABASE ALLOWS DATA COMPARISONS WITH THE WORK OF NEUROSURGEONS IN 12 OTHER COUNTRIES.EXPERT CONSULTANTS - BROUGHT AN EMERGENCY MEDICINE SPECIALIST WHO PROVIDED COURSES ON NEONATAL AIRWAY MANAGEMENT TO ALL 5 HOSPITALS, A VERY POPULAR AND WELL RECEIVED COURSE. - A UROLOGY EXPERT WHO HELPED EVALUATE CIC TRAINING, NOTED THE DIFFICULTY OF RETENTION OF PERSONNEL BECAUSE OF FREQUENT JOB ROTATIONS, AND THE LACK OF SUPPLIES.- A WOUND CARE EXPERT ASSESSED THE NEEDS IN OUR CENTERS (WHICH ARE EXTENSIVE), AND THE PROBLEMS OF RELIABLE WATER SUPPLY, POOR HYGIENE AND LACK OF GENERAL SUPPLIES.- A MEDICAL QUALITY IMPROVEMENT EXPERT WHO NOTED THAT CHART REVIEW SHOWED THAT CHART INFORMATION GENERALLY IS EXTENSIVE AND COMPLETE, BUT APPROPRIATE TESTS ARE FREQUENTLY NOT AVAILABLE AND ANTIBIOTICS USE IS PROBLEMATIC (I.E. NO AVAILABILITY OF CULTURES). - TWO DUTCH NEUROSURGEONS WHO FELT THAT SURGICAL TECHNIQUE IS VERY GOOD, SURGEON COMMITMENT EXCELLENT, AND INSTRUMENTATION IS GENERALLY SUFFICIENT.PHYSICIAN RETENTION/CAPACITY BUILDING EXPANDING THE CENTER OF EXCELLENCE MODELIN 2009 REACHANOTHER SIGNED CONTRACTS WITH 2 ADDITIONAL HOSPITALS BRINGING THE NUMBER OF CENTERS OF EXCELLENCE IN PEDIATRIC NEUROSURGERY (COE) TO 5.
