Additional Explanations

2024 accomplishment

In 2023 October, we received $100,000 from Caloptima, a public agency to hire our first employee. In 2024, we raised several grants. $105,000 Anthem for CHW capacity building, and $50000 from Caloptima for CHW capacity building. We also raised a grant funding from the California state (department of health care services) of 2.75 millon dollars PATH CITED round 3 funding, (https://www.dhcs.ca.gov/formsandpubs/publications/oc/Pages/2024/24-29-PATH-CITED-Roiund3-08-30-24.aspx) but it will not be paid until 2025. In order to receive this grant, we have to be contracted with managed care companies and provide services to low income families that is NOT medical services, not mental health. These services are include care coordination, and health education, help disabled people hire contractors to remodel their homes to be assessable, help asthma patients by buying air purifiers, humidifiers, cleaning supplies to improve their home environment to avoid emergency room visits. We registered as a medi-cal organization under the category of "community based organization ". Part of the stipulation to receive the grant funding is to be able to self sustain our mission after the grant funding ends. That is why we also have to generate income from insurance billing, by billing the public managed health plan for our services To receive this grant in 2025, we must hire people, train people to become community health workers, provide technical support like computers and cell phones, documentation software, and have offices. The population we serve also includes homeless people; we find housing for them and pay their deposit. We also serve fostered children and coordinate their medical, mental health appointments as they age out of foster home. We prepared ourselves to service justice involved individuals as they get out of jail to find housing, job and medical/mental health/drug addiction treatment. In order to train community health worker, founder Dr Shawn Hamilton volunteered her time in 2023 to meet with educators to design an asynchronous community health worker (CHW) training program. It is pending accreditation because the state of California has not published an official regulation for CHW education. We received income to provide adult school in Elk Grove with CHW training. We also train our own new hires with this curriculum. In 2024, we hired more part time and full time employee using the grant, serving 15 counties in California. We rented 10 offices in Norther California.

IRS990ScheduleO/SupplementalInformationDetail/ExplanationTxt7In 2023 October, we received $100,000 from Caloptima, a public agency to hire our first employee. In 2024, we raised several grants. $105,000 Anthem for CHW capacity building, and $50000 from Caloptima for CHW capacity building. We also raised a grant funding from the California state (department of health care services) of 2.75 millon dollars PATH CITED round 3 funding, (https://www.dhcs.ca.gov/formsandpubs/publications/oc/Pages/2024/24-29-PATH-CITED-Roiund3-08-30-24.aspx) but it will not be paid until 2025. In order to receive this grant, we have to be contracted with managed care companies and provide services to low income families that is NOT medical services, not mental health. These services are include care coordination, and health education, help disabled people hire contractors to remodel their homes to be assessable, help asthma patients by buying air purifiers, humidifiers, cleaning supplies to improve their home environment to avoid emergency room visits. We registered as a medi-cal organization under the category of "community based organization ". Part of the stipulation to receive the grant funding is to be able to self sustain our mission after the grant funding ends. That is why we also have to generate income from insurance billing, by billing the public managed health plan for our services To receive this grant in 2025, we must hire people, train people to become community health workers, provide technical support like computers and cell phones, documentation software, and have offices. The population we serve also includes homeless people; we find housing for them and pay their deposit. We also serve fostered children and coordinate their medical, mental health appointments as they age out of foster home. We prepared ourselves to service justice involved individuals as they get out of jail to find housing, job and medical/mental health/drug addiction treatment. In order to train community health worker, founder Dr Shawn Hamilton volunteered her time in 2023 to meet with educators to design an asynchronous community health worker (CHW) training program. It is pending accreditation because the state of California has not published an official regulation for CHW education. We received income to provide adult school in Elk Grove with CHW training. We also train our own new hires with this curriculum. In 2024, we hired more part time and full time employee using the grant, serving 15 counties in California. We rented 10 offices in Norther California.
