Part VI

Hospitals eligible for membership are those hospitals in district 6 that are currently participating in funding through the aspr grants program or any substitute program administered by the state or federal government. The following are participating hospitals that are granted membership: amg specialty hospital - muncie community hospital - anderson community howard regional health fayette regional health henry county community health iu health - ball memorial hospital iu health - blackford hospital iu health - tipton hospital iu health - jay hospital marion general hospital reid health rush memorial hospital st. Vincent - anderson regional hospital st. Vincent - kokomo hospital st. Vincent - mercy hospital st. Vincent - randolph hospital question 7a: each participating hospital (member) shall appoint an authorized representative to serve on the board. Question 7b: board members are representatives of the member hospitals. As such, all actions are subject to approval by the members of district 6. Question 12c: the organization has adopted a conflict of interest policy that requires directors, officers, and key employees to submit conflict of interest disclosures. The disclosure requires directors, officers, and key employees to disclose, in writing, any known financial interest that the individual (together with family members) directly or indirectly relates to any entity in the health care system of which the corporation is a part. In addition, directors, officers, and key employees are required to immediately disclose any possible conflicts of interest that arise mid year in relation to a proposed transaction. The conflict of interest policy requires that any individual with a conflict be recused from the decision-making process, that independent directors or committee members determine that the proposed transaction is in the best interest of the organization, and the transaction must be approved by a vote of the independent directors or committee members without the participation of any interested individual. Question 11, 13, 14, 15: district 6 promotes cooperation and planning for major health events as designated by the indiana state department of health. Contributions received are from governmental agencies. As such, the organization is required to follow procedures as they relate to a governmental agency. Question 19: the organization's governing documents and conflict of interest policy are available to the public upon request. The organization's financial statements are not available to the public except those provided in the form 990.
