Part VI: Hospitals eligible for membership are those hospitals in District 6 that are currently participating in funding through the ASPR Grant Program or any substitute program administered by the State or Federal Government. The following are participating hospitals that are granted membership: AMG Specialty Hospital - Muncie Community Hospital of Anderson Community Howard Regional Health Henry Community Health IU Health Ball Memorial Hospital

Question 7A: Each participating hospital (member) shall appoint an authorized representative to serve on the Board.

Governing body decisions Part VI line 7B

Question 7B: Board members are representatives of the member hospitals. As such, all actions are subject to approval by the members of District 6.

Form 990 governing body review Part VI line 11

Questions 11, 13, 14, 15: District 6 promotes cooperation and planning for major health events as designated by the Indiana State Department of Health. Contributions received are from governmental agencies. As such, the organization is required to follow procedures as they relate to a governmental agency.

Conflict of interest policy compliance Part VI line 12C

Question 12C: The organization has adopted a conflict of interest policy that requires directors, officers and key employees to submit conflict of interest disclosures. The disclosure requires directors, officers and key employees to disclose, in writing, any known financial interest that the individual (together with family members) directly or indirectly relates to any entity in the health care system of which the corporation is a part. In addition, directors, officers and key employees are required to immediately disclose any possible conflicts of interest that arise mid year in relation to a proposed transaction. The conflict of interest policy requires that any individual with a conflict be recused from the decision-making process, that independent directors or committee 12C continued: determine that the proposed transaction is in the best interest of the organization, and the transaction must be approved by a vote of the independent directors or committee members without the participation of any interested individual.

The disclosure requires directors, officers and key employees to disclose, in writing, any known financial interest that the individual (together with family members) directly or indirectly relates to any entity in the health care system of which the corporation is a part. In addition, directors, officers and key employees are required to immediately disclose any possible conflicts of interest that arise mid year in relation to a proposed transaction. The conflict of interest policy requires that any individual with a conflict be recused from the decision-making process, that independent directors or committee 12C continued: determine that the proposed transaction is in the best interest of the organization, and the transaction must be approved by a vote of the independent directors or committee members without the participation of any interested individual.
