Mission and Program Overview

Mission

Daystar's unique pediatric day-respite center programs offer regular enrichment opportunities for children in a safe and stimulating learning environment. Daystar is the only program of its kind in new york state, and enjoys close partnerships with the community's pediatric providers at golisano children's hospital, monroe county early intervention program, monroe county department of children and family services, and many other valued community partners. Daystar is licensed and governed by new york state office for people with developmental disabilities, new york state office of children and family services, and new york state department of health.

Daystar's multi-disciplinary services model combines center-based, post-acute, pediatric step-down nursing services; early childhood educational enrichment groups; individualized special education services; preschool and/or early intervention therapies; and family support services for children ages 4 months to 6 years of age. Daystar supports up to 62 enrollees daily, and is the only full-day program available to children who have complex medical conditions; require ongoing nursing supervision and care, and/or rely on medical devices to compensate for life-sustaining activities. Daystar's innovative program has been recognized by nys office of children and family services as a "best practices model and is lauded as a cost-effective alternative to home-based health care services; and is recognized as a proven prevention strategy to minimize hospitalization, and an economic catalyst, helping families preserve their economic independence and return to the workforce.

The members of the corporation reserve the right to protect and ensure the basic purpose of the corporation as described in the by-laws and the certificate of incorporation; to approve any change in the philosophy, mission, and values of the corporation; to amend the certificate of incorporation or the by-laws; to approve the disposal of all or substantially all of the assets of the corporation upon dissolution; to approve a deficit operating or capital budget; to merge, consolidate, dissolve, or otherwise change the corporate structure of the corporation; an to accept gifts which have attached conditions which impact negatively on the mission of the corporation.

The draft form 990 will be presented at a meeting of the finance and executive committee of the board of directors; the committee will be required to review and a motion to approve the form 990 will be presented. If there are changes recommended, staff will make the requested changes and present them to the committee for its final approval. Each director, officer and member of a committee with governing board delegated powers shall annually sign a statement which affirms such person: has received a copy of the conflict of interest policy, has read and understands the policy, has agreed to comply with the policy, and understands the agency's charitable mission. Board members are required to file a conflict of interest disclosure form annually; staff are trained annually on the agency's corporate compliance and conflict of interest policies, and activities are monitored quarterly through the agency's quality assurance & corporate compliance officer and committee.

Members of the agency's finance and executive committee review annual salary levels for its directors and other key employees and are required to annually review and vote upon these positions within the annual operating budget. In addition, the agency contracts with paychex, a human resources and payroll management firm. As part of the agency's annual contract, paychex provides daystar with a dedicated hr specialist, with whom the agency consults on such matters. Additionally, members of the finance/executive committee are provided with data from the annual nonprofit organizations salary and benefits survey from ny council of agency executives for comparative data.

Financial Statement Notes

PART X, LINE 2:

The organization is a non-profit organization exempt from federal and state income taxation under irs code section 501(c)(3) and applicable state tax regulations. Accordingly, no provision for federal and state income taxes has been made. The organization has filed for and received income tax exemptions in the various jurisdictions where they are required to do so. The organization files the form 990 tax return in the u.s. Federal jurisdiction and in new york state. With few exceptions, as of december 31, 2019, the organiation is no longer subject to u.s. Federal or state income tax examinations by tax authorities for years ended prior to december 31, 2017. The tax returns for years ended december 31, 2017 through december 31, 2019 are still subject to potential audit by the irs and taxing authorities in new york state. Management of the organization believes they have no material uncertain tax positions and, accordingly, they have no recognized any liability for unrecognized tax benefits.
