Form 990, Part VI, Line 11B Review of form 990 by governing body

The form 990 is prepared by a national accounting firm, based on information provided by takecare relief fund, inc. (tcrf). The form 990 is then reviewed by an officer and board member of the organization before filing.

Form 990, Part VI, Line 12C Conflict of interest policy

According to article 7 of the conflict of interest policy, a disclosure statement must be signed annually by each officer and board member. This was completed in january 18th 2024 for fye 12/31/24. The board of directors reviews the disclosure statements and determines if a conflict exists. If there is a potential conflict, the interested person shall leave the board or committee meeting while the determination of a conflict of interest is discussed and voted upon. The remaining board or committee members shall decide if a conflict of interest exists. The procedures for addressing any conflicts of interest are as follows: a. An interested person may make a presentation at the board or committee meeting, but after the presentation, he/she shall leave the meeting during the discussion of, and the vote on, the transaction or arrangement involving the possible conflict of interest. B. The chairperson of the board or committee shall, if appropriate, appoint a disinterested person or committee to investigate alternatives to the proposed transaction or arrangement. C. After exercising due diligence, the board or committee shall determine whether the fund can obtain with reasonable efforts a more advantageous transaction or arrangement from a person or entity that would not give rise to a conflict of interest. D. If a more advantageous transaction or arrangement is not reasonably possible under circumstances not producing a conflict of interest, the board or committee shall determine by a majority vote of the disinterested directors whether the transaction or arrangement is in the fund's best interest, for its own benefit, and whether it is fair and reasonable. In conformity with the above determination it shall make its decision as to whether to enter into the transaction or arrangement.

Financial Statement Notes

Schedule D, Part X, Line 2 FIN 48 (ASC 740) footnote

Accounting standards codification topic 740, accounting for income taxes, addresses how uncertain tax positions should be recognized, measured, presented, and disclosed in the financial statements. The guidance requires the accounting and disclosure of tax positions taken, or expected to be taken, in the course of preparing tcrf's tax returns to determine whether the tax positions are more-likely-than-not of being sustained by the applicable tax authority. Management of tcrf is required to analyze all open tax years, as defined by the statute of limitations, for all major jurisdictions, which include federal and certain states. Tcrf has no examinations in progress and none are expected at this time. As of december 31, 2024, management of tcrf has reviewed all open tax years from december 31, 2020 through december 31, 2024 and concluded there is no tax liability resulting from unrecognized tax benefits relating to uncertain income tax positions taken, or expected to be taken, in future tax returns.
