Form 990, Part XI, Line 9

Cpr acceptance as separate entity -125,299 in june of 2016 cardiovascular peer review llc (46-5519299) was granted tax exempt status by the internal revenue service as a 501(c)(3) organization. This enetity had previously filed on a consolidated basis with accreditation for cardiovascular excellence, inc. Cardiovascular peer review will file separately effective for this calendar year december 31, 2016. As such, all assets, liabilities and fund balance of cpr have been removed from this balance sheet. The net result decreased the fund balance by 125,299.

CARDIOVASCULAR PEER REVIEW WILL FILE SEPARATELY EFFECTIVE FOR THIS CALENDAR YEAR DECEMBER 31, 2016. AS SUCH, ALL ASSETS, LIABILITIES AND FUND BALANCE OF CPR HAVE BEEN REMOVED FROM THIS BALANCE SHEET. THE NET RESULT DECREASED THE FUND BALANCE BY 125,299.
