The CFO reviewed the Form 990 with the Board of Directors, it was reviewed by the Treasurer/Finance Committee Chair and then it was provided to the Board members for review prior to a vote.

On an annual basis, all officers, directors and key employee shall be provided with a copy of this policy and required to complete and sign the acknowledgment and disclosure form. All completed forms, as well as all other conflict information provided, shall be provided to and reviewed by the PHAB Executive Committee and be kept on file in the PHAB office.

The President/CEO's compensation was approved by the Board and Executive Committee through use of comparable data analysis provided by Human Resources Inc. (HRi), an independent management consulting firm. Other Officers' salaries were approved by the President/CEO based on the same comparable data analysis and compensation structures provided by HRi.

PHAB supports recipients to achieve several key outcomes by the end of the 5-year performance period. Ultimately, this grant will lead to accelerated prevention, preparedness, and response to emerging health threats.
