“Alliance Visiting Nurses dba VNA Alliance (VNAA) entered into a management services agreement with Concordia Community Support Services (CCSS) on 9/1/15. CCSS will perform management and policy functions and is committed to developing strong leadership so that VNAA can provide the highest quality of care from nursing and therapy to specialized services such as IV therapy, wound care, palliative care, and psychiatric care, all from the comfort of the patients' home. The management services are being provided so that VNAA can provide education and direct care to an aging population. CCSS will be responsible for appointment of the administrator for the agency and supervise all aspects of the operations of the home health program.”

There were no interest or penalties recognized on the statements of operations and changes in net assets as a result of this guidance. Generally, tax returns for years ended June 30, 2015, and thereafter remain subject to examination by federal and state authorities.”

All services are available without regard to race, religion, creed, color, age, ancestry, national origin, sex, sexual preference, lifestyle, socioeconomic status, veteran's status or handicap. The agency strives to operate in a Christian manner in all interactions with consumers, staff, and the community. The provision of care to consumers recognizes and supports consumers' rights and the inherent worth and dignity of each individual. Care is provided in a holistic, comprehensive manner that helps consumers achieve their desired quality of life and functional outcomes. Service to the community is achieved through responsive consumer care, caregiver support and education. Working as an interdisciplinary team, staff respect and support each other. Continuous quality improvement is integral to all agency functions with emphasis on employee development, risk management, strategic planning and organizational performance. The agency seeks to provide care to all consumers in need of its services while recognizing its goal to maintain financial viability. Service delivery is based on the belief that quality health care can be delivered using efficient, effective and ethical business practices. Approximately 2,531 persons were served with an average daily census of 314 patients. A total of approximately 33,857 visits (not including 513 non-billable visits) were completed by various disciplines. Caregiver assistance and support is another identified need VNA Alliance addresses by providing training and information regarding health care concerns for the geriatric and difficult-to-deal with individuals, such as those with dementia or Alzheimer's disease. We also service the corporate community through health and wellness initiatives including speaking engagements and flu clinics for agency residents and employees. VNA Alliance continues to provide a wealth of community services to the residents, organizations and various groups in Mercer, Crawford, Venango and parts of Erie, Lawrence and Butler Counties. We have gained recognition as an agency that is respected as a contributing member of the community, truly making a difference in the lives of those residing in our geographical boundaries. The focus of our community engagement is geared primarily towards educating and screening the retirement and low-income populations, as these groups seem to have the greatest need regarding health screening and health education. We work closely with individuals who provide direct hands-on care or oversee the health care of these populations as well as general potential consumers of health care services in our communities. VNA Alliance Community Liaisons have provided the following services: Blood pressure screenings for St. Paul's Colony residents. Flu clinics: Provision of 114 flu vaccines to VNA Alliance employees and patients, as well as to the public at flu clinics including St. Paul's Homes Heritage Assisted Living residents, St. Paul's Homes Greenville Ridgewood residents, St. Paul's Homes Shenango Valley Ridgewood residents, St. Paul's Homes Cottage Colony residents, Wesbury Retirement Community Assisted Living and Independent Living residents. Liaisons utilize educational materials in Meadville Medical Center and Grove City Medical Centers on CHF and COPD. Cross training educational programs to professionals in Crawford and Mercer County at the LINK collaborative at Area Agencies on Aging, as well as education on CHF and heart health on social media. The VNAA marketing team maintains a presence in the community that we serve in order to maintain effective communication with the partners and community referral sources. This includes weekly meetings, discharge planning, and therapy assessments for residents of nearby facilities.
