Statement of Program Service Accomplishments 4A

Ida residential and community based services: milestone centers, inc. (mci) provides a wide range of comprehensive residential and community-based services and supports adults with intellectual disabilities and autism (ida). Mci provides community living through 18 community-based group homes which helps individuals find comfortable homes and compatible housemates in a neighborhood setting. Each group home provides 24-hour supervision and assistance by residential direct support professionals who are trained in each individual's behavioral and medical needs. These professionals assist in meal preparation, self-care and the development of other skills. These services include transportation to day programs or work, or for leisure, healthcare, shopping and social activities. In mci's life sharing service, individuals with intellectual disabilities and autism are matched with qualified, supportive families who open their homes to them. Mci currently partners with 12 families to provide these services. Support may include transportation, self-care assistance and community integration. Families are compensated for their service and are required to receive annual training in topics such as first aid, cpr, fire safety, family dynamics, and infection control. They also are expected to be versed in mci's policies and procedures related to life sharing. Mci's community based day services serves the ida community through supported and small group employment and other day programs. The riverside day program, located in pittsburgh's lawrenceville neighborhood, assists adults with intellectual disabilities and autism in becoming as independent as possible. Focus is placed on useful skills that enable the practice of basic safety rules and community awareness. This highly individualized, planned and coordinated process with trained professionals. Activities include art, music, exercise, and community engagement. The community connections program (ccp) is a fully community-based community participation supports program that actively engages persons we support with intellectual disabilities and autism as citizens within their community. Ccp provides opportunities for individuals to volunteer for various organizations, learn about businesses, participate in wellness activities, and enjoy activities & events at various community venues. New horizon (penn hills) and garden city (monroeville) are day programs for seniors. Both are dually-licensed older adult living centers and adult training facilities. The centers are available to anyone with aging issues regardless of chronological age. The youth adventure program teaches persons with intellectual disabilities and autism outdoor and environmental skills, such as gardening, animal habitats, gardening, life skills, volunteering, responsibility and teamwork. Mci's employment programs provides assistance with resumes, job development, on-the-job training and overcoming barriers faced in the business world. Coaches also provide employer consultation and follow-up assessments of employee performance and employer satisfaction. The group employment program provides a transitional bridge to competitive integrated employment. With the support of milestone staff, small groups are welcomed into the community to provide cleaning and other services, gaining the skills and tools necessary for competitive employment within the community.

Statement of Program Service Accomplishments 4B

Mental and behavioral health: mci offers a wide range of community based programs and clinical services to assist those experiencing mental and behavioral health challenges. Community-based services includes several programs. Supportive housing provides temporary, secure apartments and living spaces for individuals 18 and older with mental health and substance use disorders to help them pursue stable, productive independent lives. Supportive housing provides on-site mentors to support residents' journey toward independent living. Our staff works closely with persons supported and their mental health service providers to identify and address barriers to housing independence, while enabling personal growth and stability. Mci's service coordination unit is responsible for helping persons supported access services in a timely manner, ensuring that services provided are appropriate and link them to available resources in the community. Mci's social rehabilitation program is a wellness service providing a therapeutic, supportive environment. Individuals supported engage in community-based or facility-based educational and recreational activities that help persons with mental health diagnoses improve social skills and interpersonal relationships, while building strong community ties. Mci's peer support program provides a unique treatment experience for service recipients. Specialized therapeutic interventions are provided by self-identified, current or former recipients of behavioral health services. These peer support specialists are certified and trained to offer support and assistance by helping others in their recovery journey and community integration. Services are provided to individuals 18 years and older that have been diagnosed with a severe and persistent mental illness. Mci's chain of hope drop-in center serves all persons supported for behavioral health and developmental disabilities in allegheny county, along with community members who desire the opportunity to engage in fellowship with others. The goal of chain of hope is to help participants re-establish links to the community through both informal activities and structured programming. Clinical services include outpatient mental health and outpatient substance abuse. Adult and youth outpatient services are provided in wilkinsburg and wilkins township. Adults and children seeking outpatient services are referred to milestone's intake department. Our intake clinicians work directly with the individual in completing an initial assessment of needs, determining insurance and fee arrangements and scheduling appointments. Individual psychotherapy is specifically designed for children, adolescents, and adults and focuses on helping them cope with problems, symptoms, and issues to better manage their lives and improve functioning. Family therapy focuses on helping the family function in more positive and constructive ways by exploring patterns of communication and providing support and education. Staff psychiatrists and nurse practitioners assess the need for medication, provide medication monitoring and discuss recommendations with the client and his/her/their family.

Statement of Program Service Accomplishments 4C

Other programs health care quality units: mci operates two health care quality units (hqcu)in the commonwealth of pennsylvania. The hcqu's provide services available to all individuals and agencies providing ida services in pennsylvania by geographic region. Services provided by the hcqu's include training courses offered by request to those who serve adults with intellectual and developmental disabilities in western pennsylvania. They provide technical assistance through consultation on matters related to adults with requiring ida services who are experiencing physical and/or behavioral health challenges. Highly-skilled clinical professionals are available to assist providers with challenges they might be facing. The hcqu's provide community education and works to identify, improve, and expand community resources to benefit people requiring ida services. The hcqu's have also developed a free app, available for android and iphone! This app contains loads of tools & resources to help you support people requiring ida services. The app contains practical information including: fatal five pocket guide easy, healthy recipes virtual fun & fitness with marena! Access to our training, our blogs, our website, and so much more!
