“Conflict of interest forms are completed annually by each board member and reviewed by the governance committee of the board. If a conflict is identified by the governance committee, they take their findings to the full board for resolution. In addition, conflict of interest forms are brought to each board meeting to insure board members have an opportunity to amend these statements at any given time. Director-level personnel statements are signed annually or as needed. Furthermore, chcb's conflict of interest expectations are outlined in the chcb employee handbook as well as the chcb code of conduct. All chcb employees - existing and new - including directors are expected to read the code of conduct and sign an acknowlegdement that is kept in their personnel file.”

“The community health centers of burlington (chcb) are leaders in providing quality care to over 30,000 patients in chittenden and southern grand isle counties, regardless of income or life circumstance. At an unprecedented time like during the covid-19 pandemic, it was especially critical for chcb to act quickly to ensure they met the needs of their patients as safely as possible. As the only federally-granted homeless healthcare program in the entire state, the area's primary location for immigrant health services, and as chittenden county's largest "spoke" in the hub-and-spoke model of addiction recovery care, chcb serves some of the area's most vulnerable populations. In the initial first months of the global pandemic caused by covid-19, their leadership teams acted quickly to reduce in-person services, and consolidate locations. They unfortunately had to furlough the majority of their dental team for two months while scaling back to emergency visits only. However, with thanks to cares funding, their dental services are now back up-and-running with improvements to the air filtration system and sterilization methods to ensure the safest and most hygienic delivery of oral health care for both patients and staff. Additionally, chcb immediately transitioned as many appointments as possible to telehealth technology. By serving patients over the phone or by video conference, they continued to expand access to patients in need while reducing the risk of cross-contamination and doing their part to "flatten the curve." chcb worked over the summer of 2020 with the city, state and nonprofit partners to operate a new drive-through testing site in south hero, and also organized a subset mobile unit van to provide testing/outreach for vulnerable populations, specifically community members experiencing homelessness. The van has allowed chcb to expand medical outreach visits to include acute and chronic disease management and preventive care, immunizations, care coordination, connection to mental health services, and covid-19 testing when clinically indicated. At the beginning of march 2021, chcb began operating a weekly vaccination clinic in collaboration with grand isle fire and rescue to ensure access to this rural community. A month later, they also started running a weekly vaccination clinic out of their riverside health center. On average, they administered 200-400 vaccines each week and are continuing their work with the vt department of health to organize other vaccination clinics in neighboring towns and businesses. Chcb remains grateful to their philanthropic donors and community partners for their stalwart support of their work and efforts through the pandemic.”

“Chcb annually files an irs form 990, return of organization exempt from income tax, tax return in the u.s. Federal jurisdiction. Chcb is no longer subject to u.s. Federal income tax examination by tax authorities for the years prior to april 30, 2018. In the normal course of business, the organization is subject to examination by various taxing authorities. Although the outcome of tax audits is always uncertain, chcb believes that there are no significant unrecognized tax liabilities at april 30, 2021.”

AS THE ONLY FEDERALLY-GRANTED HOMELESS HEALTHCARE PROGRAM IN THE ENTIRE STATE, THE AREA'S PRIMARY LOCATION FOR IMMIGRANT HEALTH SERVICES, AND AS CHITTENDEN COUNTY'S LARGEST "SPOKE" IN THE HUB-AND-SPOKE MODEL OF ADDICTION RECOVERY CARE, CHCB SERVES SOME OF THE AREA'S MOST VULNERABLE POPULATIONS. IN THE INITIAL FIRST MONTHS OF THE GLOBAL PANDEMIC CAUSED BY COVID-19, THEIR LEADERSHIP TEAMS ACTED QUICKLY TO REDUCE IN-PERSON SERVICES, AND CONSOLIDATE LOCATIONS. THEY UNFORTUNATELY HAD TO FURLOUGH THE MAJORITY OF THEIR DENTAL TEAM FOR TWO MONTHS WHILE SCALING BACK TO EMERGENCY VISITS ONLY. HOWEVER, WITH THANKS TO CARES FUNDING, THEIR DENTAL SERVICES ARE NOW BACK UP-AND-RUNNING WITH IMPROVEMENTS TO THE AIR FILTRATION SYSTEM AND STERILIZATION METHODS TO ENSURE THE SAFEST AND MOST HYGIENIC DELIVERY OF ORAL HEALTH CARE FOR BOTH PATIENTS AND STAFF.
