“Conflict of interest forms are completed annually by each board member and reviewed by the governance committee of the board. If a conflict is identified by the governance committee, they take their findings to the full board for resolution. In addition, conflict of interest forms are brought to each board meeting to insure board members have an opportunity to amend these statements at any given time. Furthermore, chcb's conflict of interest expectations are outlined in the chcb employee handbook as well as the chcb code of conduct. This partnership made sense for the long- term sustainability of their practice and also provided increased access to fqhc services, such as low-cost prescriptions and dental care, to over 6,000 patients, many of whom are elderly. -chcb's champlain islands health center began seeing patients in a newly- constructed building in december 2016. Located in south hero, this location expands access to residents of southern grand isle county. -knowing that new americans and limited english community residents often struggle with histories of trauma and complex medical conditions, chcb provided 13,386 interpreter-assisted visits to ensure culturally competent card. -this year, 357 chcb patients received suboxone treatment as part of vermont's ongoing fight against opioid addiction. -chcb's safe harbor health center underwent major renovations to increase the number of exam rooms for their homeless patients and create a larger provider workspace. A washer and dryer were installed, and there are plans to create a community kitchen space in the near future as well. -two dental operatories that were at the safe harbor health center moved to a new home at the south end health center, creating convenient access to dental services in the southernmost tip of their service area. -chcb took the lead on burlington's only low-barrier winter warming shelter, a 30-bed facility that provided safety, warm meals and connection to services for members of the local homeless population from november, 2016 through april, 2017. -chcb's school-based dental center expanded by one operatory through philanthropic giving. This site provided dental services to 638 low-income and/or uninsured children from the burlington school district.”

CHCB SERVES ALL MEMBERS OF THE COMMUNITY AS PATIENTS REGARDLESS OF LIFE CIRCUMSTANCES, INCOME OR INSURANCE STATUS. CHCB OFFERS A SLIDING-FEE SALE FINANCIAL ASSISTANCE PROGRAM FOR ALL SERVICES BASED ON INCOME WITHIN THEIR DEFINED SERVICE AREA, AND AT NO COST TO THOSE QUALIFIED AS HOMELESS. CHCB'S EIGHT HEALTH CENTERS ARE LOCATED IN CHITTENDEN AND SOUTHERN GRAND ISLE COUNTIES. THE FOLLOWING ARE A FEW OF THE FY17 HIGHLIGHTS THAT CHCB HAS ACHIEVED: -CHCB ENDED THE FISCAL YEAR PROVIDING A RECORD-SETTING 104,495 PATIENT VISITS. -CHCB MERGED WITH AN EXISTING PRACTICE IN SOUTH BURLINGTON, GOODHEALTH INTERNAL MEDICINE IN JULY, 2016. THIS PARTNERSHIP MADE SENSE FOR THE LONG- TERM SUSTAINABILITY OF THEIR PRACTICE AND ALSO PROVIDED INCREASED ACCESS TO FQHC SERVICES, SUCH AS LOW-COST PRESCRIPTIONS AND DENTAL CARE, TO OVER 6,000 PATIENTS, MANY OF WHOM ARE ELDERLY. -CHCB'S CHAMPLAIN ISLANDS HEALTH CENTER BEGAN SEEING PATIENTS IN A NEWLY- CONSTRUCTED BUILDING IN DECEMBER 2016.
