Mission and Program Overview

Mission

Acds is dedicated to providing lifetime resources of exceptional quality, innovation and inclusion for individuals with down syndrome, autism and other developmental disabilities and their families. We provide a continuum of year round programs (continued on schedule o)(continued) and services that meet the needs of children and adults and their families throughout their lifetimes. Acds provides services in three major program categories: (1) early intervention special education and therapeutic services for children from birth to age 3, (2) preschool education and related therapy services from ages 3 to 5, and (3) adult services, including individualized residential alternatives (ira's), recreational and respite programs, day habilitation program and self directed brokerage and fiscal intermediary services. We also operate a licensed day care program, as more fully described under other program services.

Major Activities

Activity 2
See schedule opreschool special educational programs acds provides preschool special education and integrated programs (in which typically developing children are also served), as well as related therapy services for children ages 3 to 5. The program is offered in a center-based classroom setting, five days a week (half and full day schedules). Emphasis is placed on the acquisition of skills in the following areas: pre-academics aligned with new york state preschool standards, language and communication, socialization, adaptive and gross and fine motor development.acds also provides home based seit services for children on the autistic spectrum in the preschool age group. Parent training is also available for this population. This year we served more than 160 children in the preschool category, in both school and home based settings. Our preschool programs were filled to capacity and 79 children graduated from our programs prepared for entry to kindergarten.
Activity 3
See schedule oadult residential servicesacds provides residential services to adults with developmental disabilities. During the year we operated 7 group homes (individualized residential alternatives or iras) for 48 individuals and one supervised transitional apartment for 2 individuals who exhibit an ability to live more independently. All of our homes provide round the clock supervision, with the goal of helping each resident progress towards independence in their daily life. The agency's group homes enable the residents we serve to leave their parents' homes and live on their own as most young adults do. Many of our residents attend day programs and work either full or part-time earning a supplemental income. Each resident's program is individualized to help them reach their fullest potential.medicaid service coordinationin addition to residential services, acds assists clients and their families in accessing medicaid, social security and social services through our medicaid service coordination department, which grew substantially again this year. Our msc's served more than 250 individuals, an increase from the prior year; this expansion included new services to the underserved bilingual population. In this capacity we help our clients to develop and implement plans to achieve their life goals, including placement in long term facilities such as ira's. Day habilitation servicesacds expanded its services to include day hab services, commencing in september 2010 with six participants. Our program, "opportuntities" has been well received in the community and was serving 28 individuals as of september 2012. Most of the participants began their education at acds and have attended acds' 5plus program; the participants volunteer at various sites in the community including our own (acds') preschool, meals on wheels, senior shopping, church outreaches, island harvest, nursing homes and horseability. Acds provides door to door transportation to our dayhab consumers.recreational and respite programsbeyond the preschool years, acds provides (through our 5plus programs) recreation and respite to individuals from age 5 through adulthood. Many individuals participate in more than one program. During the year we served 300 individuals who took advantage of over 500 program slots. The acds 5plus programs offer participants the opportunity to socialize with their peers after school, in the evenings, on the weekends and during school vacation periods. We served participants with various developmental disabilities including down syndrome, autism, cerebral palsy, william's syndrome, jacobsen's syndrome as well as a variety of other disabilities. One of 5plus' unique programs is summer adventure, an overnight respite program that is offered in 2 separate weeklong programs. In 2012/2013 this program offered 97 participants ages 16 and older the opportunity to develop independence, enjoy recreational activities and practice their daily living skills by staying in a supervised dorm setting with their friends.

IRS990/Description0SEE SCHEDULE OEARLY INTERVENTION PROGRAMSACDS PROVIDES CHILDREN WITH EARLY INTERVENTION (EI) SERVICES, FROM BIRTH TO AGE 3. INFANTS AND TODDLERS WHO DEMONSTRATE COGNITIVE DEFICITS, SPEECH AND LANGUAGE DELAYS AND/OR PHYSICAL DISABILITIES RECEIVE MULTIDISCIPLINARY EVALUATIONS, HANDS ON GUIDANCE AND INSTRUCTION FROM LICENSED SPECIAL EDUCATION TEACHERS, SPEECH AND LANGUAGE PATHOLOGISTS AND PHYSICAL AND OCCUPATIONAL THERAPISTS. EI SERVICES ARE OFFERED IN A HOME-BASED SETTING AND/OR CENTER-BASED CLASSROOM SETTING IN ACCORDANCE WITH THE FAMILY'S INDIVIDUALIZED FAMILY SERVICE PLAN (IFSP). IN TOTAL, WE SERVED MORE THAN 120 CHILDREN IN OUR EI PROGRAMS. WE PROVIDE CENTER BASED EDUCATION TO CHILDREN FROM 18 MONTHS TO 3 YEARS OF AGE WHO ARE ON THE AUTISTIC SPECTRUM USING THE ABA INSTRUCTIONAL METHOD. THE INTRODUCTION OF THE ABA INSTRUCTIONAL CLASSROOMS HAS SIGNIFICANTLY INCREASED OUR ABILITY TO SERVE AUTISTIC CHILDREN IN A CENTER-BASED SETTING AT THE EARLIEST STAGE OF SOCIAL DEVELOPMENT AND SETS THE FOUNDATION FOR CONTINUED SERVICE DURING THE PRESCHOOL YEARS. PARENTS ARE ACTIVELY INVOLVED IN DETERMINING DEVELOPMENTAL GOALS FOR THEIR CHILDREN AND WORK COLLABORATIVELY WITH TEACHERS AND THERAPISTS IN AN EFFORT TO INTEGRATE INTERVENTIONS NATURALISTICALLY INTO THE CHILD'S DAILY ROUTINE AND HOME ENVIRONMENT.
