The Form 990 is prepared by an independent CPA firm. The Form 990 is reviewed by management in detail and is provided to every member of the governing body prior to filing with the IRS.

Form 990, Part VI, Section B, line 12C

Each board member, officer, and member of senior management of the corporation, including related entities, complete an annual conflict of interest questionnaire. The signed questionnaires are reviewed by the board. Should any potential conflicts of interest be disclosed, the board member or officer would be asked to refrain from participation in any deliberation or decision with regard to matters affected by the relationship.

Form 990, Part VI, Section B, line 15

The board of directors, composed of independent persons, reviews the compensation package of the CEO/President, CFO/Treasurer, and the Chairman Emeritus/Founder using market comparability data for similar organizations. The independent board votes on the proposed compensation packages as part of the annual budget process. This process is documented in the corporate minutes and was last completed during the fiscal year.

First, they began a food distribution ministry to numerous families through food delivery to individual houses as well as mass distributions out of their Cicero location. We had 483 different kids that attend our various clubs. At the clubs, the children are given a chance to play games, do crafts, and learn about the Bible. During the year we also provide various special events for the kids that consist of special days in our building or taking kids to events. We had 0 children attend the special events in our building where we informed them about how they could become involved in our programs that we run. We also had an event we took the kids to once a month where we average 0 children per event. Each of these are focused on spending time with children while providing a fun experience for them.

The program consists of small group Bible studies, leadership training, hiring interns, helping pay for college, and helping to provide funding for full-time staff from the community. This year we had a total of 61 people that were a part of the leadership development program. We had an average of 14 kids per week in Bible study/discipleship relationship with staff, 2 kids in the beginning stages of the leadership training, 2 that work as interns, 1 student we are helping to pay for college and 4 full time staff members that were partially funded through this part of our ministry.

Camping trips range from daily getaways to week long overnight trips to a seven-week day camp program. This year we had 0 different children attend the day camp program with an average of 0 per day. We took 0 children on weeklong overnight camping trips. We took 0 children on overnight camping trips to various campgrounds. On each camping trip there were Bible lessons that were taught to the kids.

We provide selected students with scholarships to help them obtain college degrees in furtherance of this mission. The funds are paid directly to the educational institution. The ICI Food Assistance Program is set-up to assist families in need. The majority of this program is arranged around having large food distribution where food is delivered or picked up on trailer and a large distribution takes place to families in need. There are several staff and volunteers that are there that monitor how much is given to a family that is agreed up on based upon the amount of donation that is received of specific items. For each of these deliveries there is a database of the number of families served. There is also a smaller aspect that takes place of smaller quantities that is donated such as Chick-fil-a. These items are picked up on specific days by assigned staff. They are then used for the club programs and also to distribute to individual families. There is a database that is kept of deliveries to homes as well.
