The form 990 is prepared by an outside accounting firm, which is then first reviewed internally by management. Then it is provided to the finance committee of the board of directors for their review. Once the form 990 is approved, then it is made available to the entire board of directors and electronically filed.

The organization annually participates in a statewide compensation comparison survey of community health centers and uses this data to set compensation for most staff positions. The ceo and the cfo compensation is reviewed and approved by the board of director's executive committee. The ceo compensation is determined by the executive committee on an annual basis. The cfo compensation is determined by the executive committee on a semi-annual basis. The compensation process involves salary comparisons, performance reviews, and budget considerations. The compensation package is then presented for the full board of directors' approval.

During the current fiscal year, the organization spun off it's behavioral health services and long term couseling services into an unrelated tax exempt organization.

Financial Statement Notes

PART X, LINE 2:

In june 2006, the fasb issued asc topic no. 740-10, formerly fasb interpretation no. ("fin") 48, "accounting for uncertainty in income taxes, an interpretation of fasb statement no. 109" ("asc 740-10"), which clarifies the accounting for uncertainty in income taxes. Asc 740-10 prescribes a recognition threshold and measurement attribute for the financial statement recognition and measurement of a tax position taken or expected to be taken in a tax return. Asc 740-10 requires that an organization recognize in the financial statements the impact of the tax position if that position will more likely than not be sustained on audit, based on the technical merits of the position. Asc 740-10 also provides guidance related to de-recognition, classification, interest and penalties and disclosure. During the year ended june 30, 2017, the clinic performed an evaluation of uncertain tax positions and did not note any matters that would require recognition in the financial statements or which may have an effect on its taxexempt status. The clinic's income tax returns are subject to examination for all tax years ended on or after june 30, 2012 with regards to all tax positions and the results reported.
