Form 990, Part VI, Section B, Line 12C

The bridge family center has adopted a conflict of interest policy that is intended to ensure that the business of the bridge family center is conducted in a manner which merits continuous public trust and that the members of the board of directors of the bridge avoid actual or perceived conflicts of interest. The policy lays out in detail the duties of the bridge directors and defines "conflicting interest transtions". This policy is given to each new board member as part of the recruitment process. During the september board meeting, each director signs a conflict of interest statement and disclosure form. These are kept on file at the bridge family center. The statements and disclosures are reviewed with board members at the january and april board meetings to ensure that they are still complete and correct. In the event that a director proposes to engage in a conflicting interest transaction, then such director will disclose the conflict by submitting a report on the form included in the policy. The director shall furnish other information as reasonably requested. Qualified directors will deliberate the conflict. The reporting director will not participate in any deliberations. Authorization of a conflicting interest transaction shall be made by a majority vote of the qualified directors of the board of directors. If the board of directors has reasonable cause to believe a director has failed to disclose a possible conflict of interest, it shall inform the director, afford the director an opportunity to explain, and take appropriate action if there is an actual conflict.

Form 990, Part VI, Section B, Line 15A

The board president or his/her independent designee will be primarily responsible for gathering the information needed to conduct a comprehensive evaluation of the executive director's performance. The president will work in cooperation with the finance committee to recommend if a merit increase or salary adjustment is warranted and if so the amount. Information used in this process included the form 990 from three comparable local organizations, national salary data, surveyed information submitted by the bridge family center's full board and form the bridge family center's staff.

Form 990, Part VI, Section C, Line 19

The bridge family center's form 990 is reviewed in detail by the audit committee, the executive director, and the chief financial officer. All requests for additional information, clarification, and/or changes are forwarded to the bridge family center's audit firm. Once the form 990 is found to be correct and complete by the audit committee it is forwarded to the full board via email for review and questions. Once complete, the executive director will sign the document and the form 990 is submitted.

TWO ADDITIONAL PROGRAMS, ONE IN THE GREATER HARTFORD AREA AND THE OTHER STATE WIDE, HELP OLDER TEENS TO MAKE THE TRANSITION TO SELF-SUFFICIENCY THROUGH HOUSING ASSISTANCE AND LIFE SKILLS TRAINING. IN MANCHESTER, THE BRIDGE OPERATES A TRANSITIONAL LIVING APARTMENT PROGRAM THAT ASSISTS YOUNG MEN AGES 18-21 IN DEVELOPING THE SKILLS NECESSARY TO LIVE INDEPENDENTLY. THE MOSAIC PARENTLNG CENTERS IN HARTFORD AND CROMWELL (QPC) PROVIDES STRUCTURED SUPPORT AND COACHING DURING FAMILY VISITATION SESSIONS FOR PARENTS WHO ARE WORKING ON FAMILY REUNIFICATION AFTER A CHILD HAS BEEN REMOVED FROM THE HOME. IN FY25 OVER 450 YOUTHS AND FAMILIES RECEIVED ASSISTANCE THROUGH RESIDENTIAL AND PARENTING PROGRAMS.

THE BRIDGE IS LICENSED AS AN OUTPATIENT PSYCHIATRIC CLINIC BY THE CONNECTICUT DEPARTMENT OF CHILDREN AND FAMILIES AND BY THE DEPARTMENT OF PUBLIC HEALTH. BRIDGE THERAPISTS ADDRESS A VARIETY OF CONCERNS AFFECTING CHILDREN AND FAMILIES INCLUDING DEPRESSION, ANXIETY, COPING WITH DIVORCE, AND UNCONTROLLABLE ANGER.
