In the fiscal year 2024-2025, the ann silverman community health clinic (aschc) experienced the remarkable dedication and generosity of 196 individuals who collectively volunteered 5,486 hours of their time and expertise. Their invaluable contributions, estimated at a combined value of approximately $474,900 underscore the profound impact of volunteerism on our organization's ability to fulfill its mission of providing essential healthcare services to our community. Among these volunteers are healthcare professionals, including 13 physicians, 1 psychiatric nurse practitioner, 3 psychologists, 1 massage/pain relief therapist, and 8 nurses, who provided onsite care to our patients. Offsite, 87 medical specialists, including radiologists, cardiologists, and pathologists, extended their expertise to ensure comprehensive care. In our dental program, 9 community dentists and 14 onsite dentists generously provided their services, enhancing access to vital dental care. Through partnerships with organizations like vision benefits of america foundation's vision benefit program, we were able to offer eye exams and glasses to 221 patients, further expanding our reach and impact. Additionally, the dedication of our 18 board members, who contributed a total of 441 hours of service, exemplifies our shared commitment to serving the community. 7 administrative volunteers, 12 language interpreters, 3 pharmaceutical application processors, and 8 golf event volunteers contributed 3,979 hours of administrative support, valued at approximately $77,199, further demonstrating the breadth and depth of volunteer engagement at aschc. During fiscal year 2025, the ann silverman community health clinic received an estimated $514,000 in donated professional services. This amount reflects the significant in-kind contributions from volunteer clinicians, dentists, behavioral health providers, and partner organizations. Of this total, $357,587 represents donated medical services; $85,142 represents donated dental services, and $32,172 represents donated behavioral health services. In addition, penn medicine doylestown health (pmdh) provided $9,330 in donated business it support and $30,000 in donated clinical it services. These contributions were essential to the clinic's ability to deliver comprehensive medical, dental, and behavioral care to uninsured, low-income residents of bucks county. We extend our heartfelt gratitude to each and every volunteer whose selfless efforts continue to make a difference in the lives of those we serve.

Form 990, Part III, Line 4A

Medical program- ann silverman community health clinic (aschc), opened its doors in 1994 and was incorporated in 1997. It was founded 31 years ago by a local physician, dr. Dan nesi former chief of staff at doylestown hospital and his colleagues desired to provide free healthcare to community members who had limited income and were uninsured. Doylestown hospital supported their efforts to address this community's need. The clinic remains the only free clinic in bucks county. Aschc is a volunteer-driven clinic for uninsured, low-income residents of bucks county. It provides free medical care acute, chronic and preventative services as well as diagnostic testing and medication assistance to uninsured adults and children who have a household income of less than 250% of the federal poverty guidelines. In fy 24-25, aschc cared for 217 new patients at our clinic located in donated space on the ground floor of doylestown hospital. The aschc staff and volunteers utilized both telehealth and in-person clinic visits to provide the necessary care needed by our patients. There were 960 patient visits completed by our 13 volunteer medical providers -- physicians, physician specialists, and others -- who provided onsite medical services were valued at $109,594. Offsite medical specialist referrals amounted to $26,555 for total services of $136,149. A total of 98 volunteers provided medical services, including physicians, crnps, psychologists, and massage therapists in medical services, both in the clinic and offsite. The clinic also processed 585 prescriptions assistance program (pap) applications for medications needed by 436 patients, valued at $1,068,845. It is worth noting that several of the newer medications used to treat diabetes and donated by the pharmaceutical companies were quite costly in value, resulting in a significant increase in the donated value of medications this year. Aschc staff and volunteers partnered with doylestown hospital to offer vaccination clinics and aschc administered 413 flu vaccinations this fiscal year.

In the fiscal year 2024-2025, the ann silverman community health clinic (aschc) dental program continued its critical mission of addressing the pressing need for dental services among the underserved population in our community. Recognizing the profound impact of oral health on overall well-being, aschc's dental program has long been a beacon of hope for those facing financial barriers to care. With 445 visits provided by 23 dedicated volunteers, including dentists, dental assistants, and hygienists, onsite dental services valued at $21,756 were provided, alongside offsite services valued at $63,385, ensuring comprehensive dental care for those in need. Total value of all services is $85,141. Aschc's hybrid model, combining preventive services onsite with free restorative care secured through a network of volunteer dentists, has been instrumental in addressing the community's oral health needs. Since its inception in 2002, the dental program has delivered 23,676 treatment visits to 8,918 adults and children, with 639 unduplicated patients receiving care in the past fiscal year alone. Notably, 1,054 dental hygiene services were provided onsite by paid staff and volunteers, with 173 restorations completed onsite, further highlighting aschc's dedication to restoring smiles and improving quality of life. Through 1,196 total treatment visits, including both onsite and community dental visits, aschc continues to make a tangible difference in the lives of individuals and families, ensuring access to essential dental care for all members of our community. It is worth noting that due to the sporadic dental care received by many of our patients, we have implemented a quarterly scheduling system to ensure regular follow-ups and maintenance of their oral health.

Form 990, Part III, Line 4C

In the fiscal year 2023-2024, the behavioral health (bh) program at the ann silverman community health clinic (aschc) remained dedicated to providing comprehensive and compassionate care to our community members. With 4 committed volunteers, including psychologists, bh services valued at $32,172 were delivered, reflecting our unwavering commitment to addressing the mental health needs of those we serve. At aschc, we recognize that health encompasses both physical and mental well-being, and as such, our approach to healthcare is holistic and inclusive. Through annual assessments and interviews, new and returning patients are carefully evaluated to determine their eligibility for clinic services and to identify any case management needs, such as access to food, clothing, and housing. Furthermore, our patients are screened for depression and anxiety, with referrals made to our licensed professional counselor for behavioral health services as necessary. Throughout the fiscal year, a total of 684 combined social service and behavioral health visits were provided by our dedicated staff and volunteers, reflecting the breadth of support offered by aschc. Additionally, 541 individuals underwent eligibility determination visits, ensuring that our services reach those who need them most. In fy 24-25, a total of 243 counseling sessions were conducted, highlighting the importance of accessible mental health care in our community. Our volunteer psychiatric nurse practitioner, supported by interpreters, facilitated 199 behavioral health sessions, further enhancing our capacity to serve diverse populations. Patient navigator/ case management met with 442 patients and followed them for a period of two years with sessions averaging 6 times a year. Overall, 883 visits were offered in the behavioral health care program at aschc during the fiscal year, underscoring the significant impact of our bh program in improving the mental well-being of our community members.
