Mission and Program Overview

Mission

The mission of the community prevention partnership of berks county is to make a difference by offering a variety of prevention services to the community that help to address problems and risks associated with: alcohol, tobacco, and other drugs; violence; teen pregnancy; abuse; crime; school dropout; and other related social issues. The partnership carries out its mission through mobilizing communities, providing them with education, technical assistance, and resources to identify their problems and develop strategies to address them; by developing and implementing cutting edge programs and services for youth, families, and adults; and by conducting research as part of federally funded initiatives to assess the effectiveness of programs developed and delivered by the partnership.

Provide prevention services addressing the problems and risks associated with poverty, alcohol, tobacco and drug use, crime and violence, school dropout, adolescent pregnancy, childhood/adolescent obesity and other related social issues.

On january 27, 2016, community prevention partnership entered into a management and services agreement with supportive concepts for families, inc. (scffi). Effective february 29, 2016, all employees of the community prevention partnership became employees of scffi.

All employees and members of the board sign a conflict of interest policy. The policy requires disclosure of interests that could cause conflicts at any and all times.

The board of directors has approved a policy for evaluating the executive director of the cpp. It includes a review of the accomplishments of the executive director based on the approved goals and objectives set the prior year, approval of the goals and objectives for the upcoming/current year, and a recommendation for a salary increase if the executive director has had satisfactory performance. This process is first completed by the executive/personnel committee of the board of directors and then its recommendation is presented to the full board of directors for discussion, review, and approval.

TRAINED PROFESSIONALS CONDUCT HOME VISITS PROVIDING INFORMATION, SUPPORT AND ENCOURAGEMENT TO PARENTS DURING THE CRUCIAL EARLY YEARS OF LIFE. THE PROGRAM PROVIDES MENTORING FOR 225 LOW-INCOME FAMILIES WITH CHILDREN AGED 0 - KINDERGARTEN. IT IS FUNDED THROUGH A GRANT FROM THE PENNSYLVANIA DEPARTMENT OF HUMAN SERVICES.

THE PROGRAM IS A CHILDHOOD OBESITY PREVENTION PROGRAM FOCUSING ON LATINO FAMILIES WITH CHILDREN AGES 2-6 YEARS. BILINGUAL HOME VISITORS VISIT LATINO FAMILIES RESIDING IN READING, PENNSYLVANIA AND DELIVER A SERIES OF 11 HEALTH, NUTRITION AND WELLNESS FOCUSED LESSONS. EACH LESSON IS ACCOMPANIED BY AN AGE-APPROPRIATE ACTIVITY. MONTHLY GROUP SESSIONS PROVIDE FAMILIES WITH OPPORTUNITIES FOR SOCIAL OUTINGS FOCUSED ON PROMOTING PHYSICAL FITNESS. THE PROGRAM HAS SERVED 60 FAMILIES WITH DIRECT SERVICES AND COLLECTED COMPARISON DATA ON 60 ADDITIONAL FAMILIES. KIDS AGAINST PRESSURE PROVIDES SKILL BUILDING TO APPROXIMATELY 120 CHILDREN IN AN AFTER-SCHOOL PROGRAM THAT SERVES LATINO STUDENTS IN FIVE ELEMENTARY SCHOOLS IN READING. THE PROGRAM FOCUSES ON ENHANCING SELF-ESTEEM WHILE RECOGNIZING AND ACCEPTING INDIVIDUAL LIMITATION AND DEVELOPING SOCIAL SKILLS TO BUILD POSITIVE PEER RELATIONSHIPS. THE PROGRAM IS FUNDED BY A CONTRACT WITH SERVICE ACCESS AND MANAGEMENT, INC. THROUGH THE BERKS COUNTY OFFICE OF MENTAL HEALTH/DEVELOPMENTAL DISABILITIES.CIRCLE OF FRIENDS DROP-IN-CENTER SERVES APPROXIMATELY 245 ADULT CONSUMERS OF MENTAL HEALTH SERVICES AND PROVIDES THEM WITH SOCIALIZATION AND SUPPORT SERVICES. THE PROGRAM OFFERS MENTAL HEALTH EDUCATIONAL, RECREATIONAL, AND SOCIALIZATION OPPORTUNITIES. MEMBERS ALSO RECEIVE ASSISTANCE IN ACCESSING BASIC SERVICES. THE PARENT PARTNER PROGRAM ASSISTS IN BUILDING BRIDGES OF TRUST BETWEEN THE PARTICIPANTS AND THE SERVICES DESIGNED TO MEET THEIR NEEDS. THE PROGRAM IS FUNDED BY A CONTRACT WITH SERVICE ACCESS AND MANAGEMENT, INC. THROUGH THE BERKS COUNTY OFFICE OF MENTAL HEALTH/DEVELOPMENTAL DISABILITIES.EDUCATION LEADING TO EMPLOYMENT AND CAREER TRAINING IS A CONTRACTED PROGRAM WITH THE BERKS COUNTY INTERMEDIATE UNIT THAT ASSISTS PREGNANT AND PARENTING TEENS IN BERKS COUNTY TO STAY IN SCHOOL, EARN A HIGH SCHOOL DIPLOMA OR GED, AND MAKE THE TRANSITION TO EMPLOYMENT. THROUGH FUNDING FROM THE COUNCIL ON CHEMICAL ABUSE, THE PARTNERSHIP PROVIDES SUPPORT AND RESOURCES TO COMMUNITY GROUPS FOCUSING ON YOUTH SUBSTANCE ABUSE AND EDUCATION TO NURSE FAMILY PARTNERSHIP AND KIDS AGAINST PRESSURE PROGRAM PARTICIPANTS ON THE RISKS OF OTC DRUGS, MARIJUANA AND OPIODS.THE UNITED WAY VENTURE GRANT WAS AWARDED TO PROVIDE NUTRITON/HEALTHY LIFESTYLE LESSONS TO PARTICIPANTS IN THE PARTNERSHIP PROGRAMS. THE BERKS COUNTY COMMUNITY FOUNDATION AWARDED TWO GRANTS TO ENHANCE EXISTING PROGRAMS. THE BRIGHT SMILES GRANT WAS AWARDED TO PROVIDE DENTAL EDUCATION AND SUPPLIES TO PARTICIPANTS IN VARIOUS PROGRAMS. THE FEEDING AND NURTURING FAMILIES GRANT WAS AWARDED TO PROVIDE FOOD TO NEEDY FAMILIES WHO ARE SERVED.THE UNITED WAY OF SCHUYLKILL COUNTY COMMITTED $10,000 OF SUPPORT FOR THE NURSE FAMILIY PARTNERSHIP PROGRAM.
