Governance Explanations

Form 990, Part VI, Line 15 COMPENSATION REVIEW

The process for determining compensation for the heath center's top management official, other officers, and key employees was led by the health center's board of directors over a year-long process in 2019. This process included several reviews by the full board, comparability data, and detailed, position-by-position review. The process resulted in an organization wide salary guide for fiscal year 2019-2020 which has been implemented going forward. This guide includes a salary range by position which accounts for experience and qualifications. These discussions are documented in meeting minutes. Subsequent changes in salary have been based on organization-wide adjustments.

Form 990, Part VI, Line 11B Review of form 990 by governing body

The form 990 is prepared by an independent accounting firm based on the audited financial statements and information provided by the accounting department of the organization. The chief financial officer reviews the return with the center's finance committee, whose members include the board treasurer, top management official, and chief administrative officer, chief executive officer and chief operating officer. Following this review, the prepared form 990 is then distributed to the board of directors prior to filing the return with the irs.

Form 990, Part VI, Line 12C Conflict of interest policy

The conflict of interest policy is distributed to officers, board members and key employees annually. The health center's board of directors considers possible conflicts of interest of each board member, officer and key employee in each meeting depending upon the subject at hand. When deemed necessary, board members are required to recuse themselves from discussions and/or abstaining from voting regarding any matter involving possible conflicts of interest. Family and business relationships as sources of potential conflicts are expressly mentioned in the conflict of interest policy.

OUR THREE HEALTH CENTER SITES AND WELLNESS CENTERS IN NORTH PHILADELPHIA (FIFTH STREET, HUNTING PARK, AND KENSINGTON) ARE CONVENIENTLY LOCATED FOR EASY ACCESS. SERVICES INCLUDE PRIMARY MEDICAL CARE FOR ADULTS AND CHILDREN, DENTAL CARE, PRENATAL CARE, BEHAVIORAL HEALTH SERVICES, ON-SITE MEDICATION DISPENSARIES, SOCIAL SERVICES ASSISTANCE, AND OTHER SERVICES. COMMUNITY HEALTH AND WELLNESS PROGRAMS ARE OPEN TO ALL RESIDENTS. ESPERANZA HEALTH CENTER IS RECOGNIZED AS A FEDERALLY QUALIFIED HEALTH CENTER (FQHC) BY THE U.S. DEPARTMENT OF HEALTH AND HUMAN SERVICES, HEALTH RESOURCES AND SERVICES ADMINISTRATION, BUREAU OF PRIMARY HEALTH CARE. DURING THE CALENDAR YEAR 2024, EHC DELIVERED PRIMARY CARE SERVICES TO MORE THAN 14,848 PATIENTS. THESE PATIENTS ACCOUNTED FOR 49,918 MEDICAL VISITS IN FAMILY PRACTICE, INTERNAL MEDICINE, PEDIATRICS, OBSTETRICS/GYNECOLOGY, AND ORTHOPEDICS. IN ADDITION, THERE WERE 10,028 BEHAVIORAL HEALTH VISITS, AND 7,000 DENTAL VISITS. APPROXIMATELY 61% OF PATIENT SERVICES REVENUE WAS REIMBURSED THROUGH MEDICAID INSURANCE COVERAGE, 14% BY MEDICARE, 5% BY PRIVATE/COMMERCIAL INSURANCE AND SELF-PAY FROM PATIENTS WHO HAVE NEITHER PUBLIC OR PRIVATE INSURANCE COVERAGE, AND 20% FROM 340B MEDICATION DISPENSARY. ALL OF EHC'S PATIENTS WITHOUT INSURANCE COVERAGE ARE ELIGIBLE FOR A DISCOUNT FOR THE PATIENT'S SHARE OF FINANCIAL RESPONSIBILITY, AS DEFINED BY EHC'S SLIDING FEE DISCOUNT SCALE.
