Form 990, Part VI, Section B, Line 15

The ceo is formally evaluated annually by the executive committee of the board of directors on her anniversary date. All other key employees are reviewed annually by their immediate supervisor on their anniversary date. This process utilizes a standardized form of general supervisory and management skills. In addition each employee receives a development plan and is assigned specific goals from the organizational strategic plan which are evaluated in subsequent years.

Form 990, Part VI, Section C, Line 19

Mfhs will make available copies of the organizations financial statements, conflicts of interest policy and other governing documents upon receiving a formal written request for such documents. Mfhs does not currently charge for such requests.
