“The Form 990 is prepared by the Chief Financial Officer using information obtained from electronic records of Foulkeways at Gwynedd and personal interviews with the Chief Executive Officer and various members of the management team as needed. Upon completion of the document, the Form 990 is distributed in its entirety to the Finance Committee for review and comment. After the Finance Committee reviews the return, it is distributed to the full Board of Directors. It is then filed electronically with the Internal Revenue Service as required.”
“At the January Meeting of the Board of Directors, each board member is requested to complete a Conflict of Interest Statement disclosing any conflicts that may exist. The Conflict of Interest Statement is signed by the board member and then reviewed by the Board Chair and the Chief Executive Officer. If, during the year, any decision is proposed related to an area where there is a conflict, the board member with the conflict is expected to recuse themselves from the decision. A similar Conflict of Interest Statement is also signed by the organization's operating officers and key employees.”
“The Foulkeways Board of Directors and the Chief Executive Officer (CEO) Evaluation Committee (comprised of the Board Chair, Vice Chair and Chair of the Pension/Personnel Committee) evaluate the CEO on an annual basis utilizing information from board members and other sources. The Board Chair gathers information both verbally and written from Board Members regarding the performance of the CEO. The Board Chair also receives a recap of the previous year's goals and the goals for the upcoming year from the CEO. The CEO Evaluation Committee meets and reviews all of the information. Subsequently, a meeting is held with the CEO to review and discuss the information and to establish goals for the coming year. A recommendation for compensation is then presented to the next full Board of Directors meeting for final approval.”
“Foulkeways makes the governing documents and conflict of interest policy available to those who inquire. These are available from the Administrative Assistant to the Chief Executive Officer in the Administrative offices on the Foulkeways campus. Foulkeways produces an Annual Report each year that includes the complete audited financial statements. The Annual Report is distributed to all residents and priority list members. It is also given to employees and other interested parties as requested. The Annual Report is included in the information packet that is given to those individuals requesting information about the community. The Financial Statements are posted to the NRMSIR for use by bondholders of the Community. They are also posted on Guidestar.com as part of the Form 990 posting for not-for-profit charitable organizations.”

| IRS990/Desc | 0 | Health Care Service Program: The basis for all Foulkeways Health Services is a primary care model for those over 65 consisting of health maintenance, education for self-care, appropriate screening, management of acute illness, monitoring of chronic disease and achievement of dignified death. The health and wellness needs of residents are addressed at all levels of care. Most residents receive primary care from onsite physicians who are board certified in geriatric medicine. HEALTH PROMOTION AND MAINTENANCE: Residents at any level of care may take part in the Wellness Program. The Fitness Center, including an indoor 65' by 25' pool (equipped with chair lifts for handicapped accessibility) and a fully equipped exercise facility is available to all residents, priority list members and staff. Independent Living residents are considered able to demonstrate their independence in all Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs). Personal Care (Abington House) - Residents require minimal to moderate assistance with Activities of Daily Living. Nursing Facility (Gwynedd House) - the majority of nursing care residents are in their 90's and often have a strong preference to be treated with less invasive technologies at Foulkeways. STAFF EDUCATION AND TRAINING: Required regulatory seminars are provided annually either in person, using a Powerpoint deck or through electronic training. Ongoing educational opportunities are provided on a monthly basis to nursing staff. |
