“Madison day care center (mdcc) was aquired by family service of morris county (fsmc), a non-profit corporation located in morriston, new jesey, on february 8, 2011. Fsmc changed its name to cornerstone family programs (cfp) during 2013. Mdcc changed its organizational structure with fsmc/cfp becoming the sole member of the corporation. This acquisition was accounted for under the purchase method of accounting for $3,104 in 2011. Mdcc continued to provide day care services for children in madison, new jersey, through march 24, 2015, the date that cfp, its sole member, voted to dissolve mdcc. There were no assets involved in the entity as of the date of dissolution per the new jersey certification of dissolution, april 6, 2015. The day care is now being run by a separate, unrelated non-profit organization. Fsmc presents a consolidated financial statement which includes activities for mdcc through april 6, 2015. This financial information is included on the fsmc form 990. The mdcc entity has no activity.”
