“The Form 990 is prepared by VSA based on narrative and financial information provided by the Executive Director and Treasurer of the Board. Draft copies are made available to the ED and Treasurer, who meet and discuss. The process for review includes a full read of the draft, number tie-outs, and a question period between the preparer and the organization. The draft is then provided to the Board President, who reviews and provides comments. After a new draft is provided, it is shared with the full Board. The comment period is two weeks, and at that time final changes are given to VSA for final issuance and filing of the Form 990.”

“Board members are required to provide evidence of review of conflict of interest policy annually via signed form. Any conflict that arises is required to be disclosed immediately. Compliance is monitored through this process, and discussed regularly in Board meetings.”
