“Transforming clinical practice initiative (tcpi) grant: tcpi is a federal grant program that was awarded to arizona health-e connection in 2015. The practice innovation institue (pii) will prepare and enable 2,500 arizona providers for successful participation in value-based, alternative payment arrangements. The pi institute is a partnership of azhec, mercy care plan and mercy maricopa integrated care. Hie subsidy program: support the participation of eligible hospitals, federally-qualified health centers (fqhcs), rural health clinics (rhc) and eligible providers in bidirectional health information exchange through the hie. Goal of the hie is to improve care coordination, reduce duplicative treatments and improve health outcomes. Statewide hie intregration plan (ship) program: supports the participation of eligible behavioral health entites in bidirectional health information exchange through the hie. This program facilitates the integration of physical and behavioral health information in the hie to improve care coordination, reduce duplicative treatments and improve health outcomes.”
