College success foundation is the sole member of washington state mentors.

The sole member, college success foundation, appoints its board chair, chief operating officer and board secretary to the washington state mentors board.

Form 990, Part VI, Section A, Line 7B

The sole member approves the election of the washington state mentors board members. Upon dissolution of washington state mentors, the sole member's board members have the responsibility of distributing the remaining assets of wsm.

Form 990, Part VI, Section B, Line 11

The review process includes a detailed review by the controller and the interim cfo of college success foundation (controlling entity), along with the president. The form 990 is also reviewed by college success foundation's board finance committee, including the chairperson of the csf board. Subsequent to the review by the finance committee and prior to the filing of the return, all board members receive a copy of the 990 for review.

Form 990, Part VI, Section B, Line 12C

The policies, rules, and regulations adopted by the corporation relating to conflict of interest shall apply to any person appointed by the board of directors in any capacity and to the members of the board of directors themselves and key employees. The conflict of interest questionnaires are requested of all directors annually and for new directors at the time they join the board. The cfo reviews the questionnaires and if a conflict arises, the board member recuses themselves from the discussion and vote.

The chief executive officer's salary is determined by recommendation from the college success foundation's governance committee to the personnel and compensation committee and voted on by the csf board of directors. External market salary reviews are conducted for all positions, including the ceo's, every 3-4 years. A review of the ceo position was completed and presented to the personnel and compensation committee in august 2013. The executive director and cfo's salary is reviewed by college success foundation's personnel and compensation committee and approved by the csf board of directors. If the csf board approves increases for all staff, the ceo and csf board chair review each individual in the administration process of increases. Compensation increases for the executive director and cfo was last approved at the january 2013 board meeting.

THESE FORMAL PROGRAMS SERVED 32,000 YOUTH FROM AGES 5-21 ACROSS THE STATE. WSM OUTREACH ALSO INCLUDED CONTINUED SUPPORT TO NINE COUNTY-WIDE COALITIONS OF YOUTH MENTORING PROGRAMS THAT MET MONTHLY TO ADDRESS CHALLENGES AND GENERATE LOCAL SOLUTIONS TO THE CHALLENGES. WSM CONVENED MEETINGS FOR AFFINITY GROUPS FOR COLLABORATIVE EFFORTS, E.G., MENTORING AFRICAN AMERICAN BOYS. IN ADDITION, WSM PARTNERED WITH WASHINGTON'S OFFICE OF THE SUPERINTENDENT OF PUBLIC INSTRUCTION TO COLLABORATE IN A CALL-TO-ACTION CAMPAIGN FOR DROPOUT PREVENTION AND MENTORING.TRAINING: WSM HELD A STATEWIDE YOUTH MENTORING CONFERENCE ATTENDED BY 163 PEOPLE THAT INCLUDED BREAK-OUT SESSIONS DESIGNED TO TRAIN PROGRAM STAFF IN BEST PRACTICES AND CORE ISSUES SUCH AS CULTURAL COMPETENCY AND YOUTH RISK. WSM ALSO PROVIDED 10 HALF- AND FULL-DAY TRAININGS THAT INCLUDED 36 DIFFERENT MENTORING PROGRAMS (478 PARTICIPANTS) WITH CONTENT RANGING FROM BEST PRACTICES, DEALING WITH TRAUMA-AFFECTED YOUTH.TECHNICAL SUPPORT: WSM PROVIDED TECHNICAL SUPPORT TO 3 NEW PROGRAMS REGARDING PROGRAM STRUCTURE, DESIGN AND OUTCOME EVALUATION. WITHIN THE QUALITY ASSURANCE AND CONTINUOUS IMPROVEMENT PROGRAM (DISCUSSED UNDER RESEARCH), WSM STAFF WORKED CLOSELY WITH SIX PROGRAMS TO IMPROVE ORGANIZATIONAL OPERATIONS (RISK MANAGEMENT AND STRATEGIC PLANNING), BEST PRACTICES (MATCH CLOSURE AND FAMILY PARTICIPATION) AND EVALUATION (HOLISTIC EVALUATION PLANNING AND MENTOR OUTCOMES). WSM CONTINUES TO SUPPORT JUVENILE REHABILITATION SERVICES AND CHILDREN'S ADMINISTRATION WITH THEIR MENTORING PROGRAMS.RESEARCH: WSM'S RESEARCH DOMAIN ALSO INCLUDES EVALUATION AND QUALITY ASSURANCE. WSM'S QUALITY MENTORING ASSESSMENT PATH (QMAP) IS A STATEWIDE INITIATIVE DESIGNED TO INCREASE THE QUALITY OF PROGRAMS AND DEEPEN THEIR USE OF THE ELEMENTS OF EFFECTIVE PRACTICE FOR MENTORING. THE QMAP WORK IS PART OF NATION-WIDE PROCESS EVALUATION FUNDED THROUGH WSM'S NATIONAL AFFILIATE MENTOR. IN 2013, 6 PILOT PROGRAMS PARTICIPATED AND 23 NEW PROGRAMS APPLIED TO ENTER THE QUALITY ASSURANCE PROCESS IN 2014 AND 2015. IN 2013, WSM LED THREE RESEARCH AND DEMONSTRATION PROJECTS AND SUBMITTED A MULTI-SITE GRANT TO THE DEPARTMENT OF EDUCATION IN CONCERT WITH ACADEMICS FROM THREE MAJOR AMERICAN UNIVERSITIES (MENTORING STUDENTS OF DEPLOYED MILITARY PARENTS). THE MENTORING FOR ACADEMIC PERFORMANCE PROJECT WAS DESIGNED IN 2013 FOR IMPLEMENTATION AND OUTCOMES ANALYSIS IN 2014. A COMPREHENSIVE SURVEY OF ALL MENTORING ACTIVITY IN THE SOUTH SEATTLE COLLECTIVE IMPACT AREA WAS COMPLETED TO IDENTIFY THE TYPES OF ACTIVITIES LABELLED MENTORING AND WHAT YOUTH ARE INVOLVED AND WHAT SERVICES THEY RECEIVE.DISSEMINATION OF A 5-YEAR STUDY REPORT, THE ROLE OF RISK OCCURRED THROUGH WEBINARS, PRESENTATION TO MENTORING PROGRAMS IN WASHINGTON, NEBRASKA, AND MINNESOTA AND AT THE NATIONAL CONFERENCE SPONSORED BY THE NATIONAL AFFILIATE MENTOR. WSM MADE THE YOUTH RISK SURVEY AVAILABLE TO THE PUBLIC VIA OUR WEBSITE. THE STUDY REPORT WAS DISSEMINATED AND RE-DISSEMINATED THROUGH SOCIAL MEDIA TO OVER 850,000 CONTACTS; OVER 2,000 COPIES OF THE EXECUTIVE SUMMARY HAVE BEEN DISTRIBUTED WITH THE FULL REPORT BEING DOWNLOADED OVER 1,500 TIMES. IN ADDITIONAL TO AN OVERVIEW OF THE STUDY, THE PRESENTATIONS HIGHLIGHT THE STRONG FINDING THAT YOUTH IN 1:1 COMMUNITY-BASED MENTORING HAD REDUCED SYMPTOMS OF DEPRESSION. THIS IS ESPECIALLY IMPORTANT SINCE 22% OF THE YOUTH IN THE GENERALIZABLE STUDY HAD SEVERE SYMPTOMS OF DEPRESSION.
