Each board member reeives a draft copy of the Form 990 in advance of the due date. They are asked to review the form and to ask quetions regarding the information contained on the form.

Conflict of interest policy compliance Part VI line 12C

Each person covered by the conflict of interest policy is requied to provide an annual update to the Board of Directors indicating their compliance with the policy. The person is also asked to review and sign a oopy of the Conflict of Interest policy annually.

Mid-Valley follows IRS guidelines and uses an independent compensation committee of the Board of Directors whose responsibility is to review and approve the compensation arrangements for certain key employees. A voting member of the Board who receives compensation from Mid-Valley is precluded from voting on matters pertaining to that members compensation.

Our offerings include case management, day programming and residential services. We offer residential services at 3 separate locations within our service area. These residential settings enable our patients to integrate themselves into their communities. IRS990/ProgSrvcAccomActy2Grp/Desc0Mental health services to individuals with mental illness and substance abuse issues. We offer a wide range of programming to patients including outpatient services, community support services, residential services, crisis stabilization services, day programming, case management, psychology and psychiatric services. Our service area is located in Wetzel County West Virginia and operates out of two primary clinical sites in New Martinsville, WV. Mid-Valley Healthcare provides services to a variety of patients most of which are covered by Medicaid. Many of our patients that are not covered by Medicaid qualify for free care through our charity care policy. In fiscal year 2016, Mid-Valley nearly $356,000 in charity care. A large portion of our service area is rural and we are able to provide our patients access to a psychiatrist and other services via telemedicine.
