(s)he (a) received a copy of the conflicts of interest policy; (b) read and understands the policy; (c) agreed to comply with the policy, and (d) understands leaf is a not-for-profit organization engaged in activities consistent with its purpose and mission.”
“The leadership for education achievement foundation's policy is to fully comply with the federal (and any state) public disclosure requirements with respect to irs forms 990, 990-t, 1023, and 5227. The council will fulfill all requests for the forms made in writing or in person. If a request is made in writing, leaf must mail the copy within 30 days from the date it receives the request. If in-person request, at its principal offices located at 7 elk street, albany, new york during regular business hours (8:30am to 5:00pm monday-friday). The organization must provide the copies to a requestor on the day the request is made. However, when unusual circumstances exist so that fulfillment of the request on the same business day places an unreasonable burden on leaf, the copies must be provided on the next business day following the day on which the unusual circumstances cease to exist or the fifth business day after the date of the request, whichever occurs first.”
