Financial Statement Notes

Part X, Line 2:

The Center is organized as a North Dakota nonprofit corporation and has been recognized by the Internal Revenue Service (IRS) as exempt from federal income taxes under Section 501(a) of the Internal Revenue Code as an organization described in Section 501(c)(3), qualifies for the charitable contribution deduction under Section 170(b)(1)(A)(vi), and has been determined not to be a private foundation under Sections 509(a)(1). The Center is annually required to file a Return of Organization Exempt from Income Tax (Form 990) with the IRS. In addition, the Center is subject to income tax on net income that is derived from business activities that are unrelated to their exempt purposes. The Center is not subject to unrelated business income tax and has not filed an Exempt Organization Business Income Tax Return (Form 990-T) with the IRS. The Center believes that it has appropriate support for any tax positions taken affecting its annual filing requirements, and as such, does not have any uncertain tax positions that are material to the financial statements. The entity would recognize future accrued interest and penalties related to unrecognized tax benefits and liabilities in income tax expense if such interest and penalties are incurred.

IRS990/Desc0The Behavior Service Program is a specialized service dedicated to improving the lives of children and families in the community. The Behavior Service Program consists of two different services: ABA Therapy and Community Support. Each child in the ABA Therapy completes an interactive assessment that looks at a child's skill set across a wide variety of language and learning skills. The results of the assessment are used to create a road map to help children in the program achieve success in all aspects of their life. Individualized evidence strategies are then used to help propel children to reach their goals in a fun, interactive, supportive, and knowledgeable atmosphere. The ABA Therapy service is supervised by a Licensed Board Certified Behavior Analyst. The Community Support service focuses on helping children achieve success across a variety of community settings (i.e. home, school, grocery store, car rides, bus rides, doctor office, dentist office, etc.). Each family member/teacher/community professional seeking consultation in the Community Support service works with a Licensed Board Certified Behavior Analyst to determine where behavior support is needed and to set goals for success. The Behavior Analyst then works with the family to set up the supports, training, programming, and equipment necessary to help each and every child work toward their goal. The Behavior Services Program provided 16,222 hours of service for 51 children and 4 school districts in 2015.

The program has options for full-day, half-day, before school, after school and summer enrollment and offers a 1 staff to 3 client ratio which is crucial in maintaining a successful environment. The program is divided into age groups and follows age appropriate daily lesson plans designed specifically for the children served. The program uses positive behavior support principles and procedures, which seek to support and reinforce appropriate behavior to replace negative, problem behaviors. Staff members are trained to assist in the development of multiple skills that include, but are not limited to, functional communication, appropriate play, social interaction and self-help skills. The AuSome Kids Day Program provided 30,849 hours of service for 47 children in 2015.

IRS990/ProgSrvcAccomActy3Grp/Desc0The In-Home Supports Program (IHS) is a service that provides support to individuals on the Developmental Disabilities Waiver of ND. The IHS program benefits the primary caregiver by assisting the individual in activities of daily living such as eating, drinking, toileting, and physical functioning; improving and maintaining mobility and physical functioning when these tasks require more than one person to accomplish. If no other natural support is available the program may also include assisting the individual with maintaining health and personal safety while the primary caregiver is home and attending to other household tasks or children. The IHS can be provided to the individual while the primary caretaker is home or away from home, but is unavailable to care for the individual. The team, which includes the primary caretaker, determines the appropriate tasks or activities provided during the support hours and is included in the individuals' plan. The IHS program is intended to help the family live life to the fullest by supporting the needs of the individual and family with the intent of preventing or delaying out of the home placement. The NDAC provided 16,100 hours of In-Home Supports in 2015. The program grew from 22 clients at the beginning of the year to 33 clients by the end of 2015. The program is accredited by the Council on Quality and Leadership.

Our goal is to provide child(ren) with a clean safe, comfortable environment where they can play and learn with guidance, loving care and support. The Center has experienced and well-trained staff that are committed to the goals of providing support to children and enriching the social, physical, creative, and intellectual experiences for children of all ages and needs. All employees at the Center are supervised by and are responsible to the Director of Child Care Services. A training and orientation period is required for employees and volunteers. The Puzzle Place Childcare Center's curriculum is developmentally appropriate for each classroom. Some focus areas of the curriculum are creativeness, sensory, cognitive, physical, language, social, and emotional development. There is a new theme each week and activities planned around those weekly themes. We encourage parent involvement and feedback in all aspects of our center. The Puzzle Place Childcare Center provided a total of 125,167 hours childcare for 62 children.

The Center is annually required to file a Return of Organization Exempt from Income Tax (Form 990) with the IRS. In addition, the Center is subject to income tax on net income that is derived from business activities that are unrelated to their exempt purposes. The Center is not subject to unrelated business income tax and has not filed an Exempt Organization Business Income Tax Return (Form 990-T) with the IRS. The Center believes that it has appropriate support for any tax positions taken affecting its annual filing requirements, and as such, does not have any uncertain tax positions that are material to the financial statements. The entity would recognize future accrued interest and penalties related to unrecognized tax benefits and liabilities in income tax expense if such interest and penalties are incurred.
