Form 990, Part VI, Section A, Line 6

Father Flanagans Boys Home FFBH is the sole member of all subordinate organizations. Dissolution of any of the subordinate organizations shall occur only upon the approval of FFBH, in which event its net assets shall be distributed to FFBH.

Form 990, Part VI, Section A, Line 7A

Father Flanagans Boys Home, the sole member, hires/appoints the organizations executive director. According to the by-laws of the organization, the executive director is the president and a voting member of the board.

Form 990, Part VI, Section A, Line 7B

FFBH, the sole member, must approve the appointment or removal of subordinate organizations directors. All subordinate organizations operate under an affiliation agreement with FFBH that controls all of their activities. Dissolution of a subordinate organization shall only occur upon the approval of FFBH, in which event the net assets of the subordinate organization, shall be distributed to FFBH.

Form 990, Part VI, Section B, Line 11B

A review was initially performed by an external accounting firm. Then the organizations treasurer and president were provided a copy for review. After these reviews, an electronic copy of the final Form 990 was provided to all directors of the affiliate board before it was filed.

Form 990, Part VI, Section B, Line 12C

FFBH regularly and consistently monitors and enforces compliance with its conflict of interest policy mainly through official annual affirmations, self-reporting and observation. Directors are covered by a board of trustee policy, and a separate policy covers officers and employees. Directors must report a perceived or actual conflict of interest to the Chairman of the Boards Executive Committee and has no vote in determining whether a conflict exists. A board member may be disqualified from participating in certain deliberations and votes during and after a review and may be required to resign if a conflict exists. Officers and higher level employees are required to and any other employee may at any time report any situation involving a conflict of interest to their Associate Executive Director and or the Legal Department for review and determination as to how to proceed. Any perceived conflict of interest can also be reported for review through a confidential organizational ethics line at www.boystownethics.com

Victor LaPuma and Jeffrey Fusselman provide services to Father Flanagans Boys Home and all affiliates, therefore the remaining hours weekly is spent in that capacity. None of Victor LaPumas salary is allocated to affiliates.

New York certified teachers and Boys Town New York, Inc. staff partner to provide students with a positive, structured, and mainstream-learning environment.

Form 990, Part III, Line 4C

Services focus on the entire family. Consultants also help families learn how to solve problems that may threaten their stability or the childrens safety after the intervention ends. Boys Town New York, Inc. also provides other supportive services to families. The goal of these services is to help keep families intact they may be focused on promoting safety, providing opportunities for family visitiation, and/or connecting families with resources in their communities.

Form 990, Part V, Line 2A

Father Flanagans Boys Home FFBH is the sole member of all subordinate/affiliate organizations. As part of the affiliate agreement with FFBH, all affiliates are to utilize FFBH employees to perform the mission of the organization. Costs incurred by FFBH for these employees are then passed on to the affiliate sites. Due to this arrangement, salaries are reported as a cost to the organization, but no W-2s are filed in the name of the affiliate organization.

Form 990, Part XII, Line 3B

Boys Town New York, Inc. is included in the A-133 audit that is completed on a consolidated basis for FFBH and its affiliates. There is not a separate A-133 audit for the monies awarded solely to Boys Town New York, Inc.

Boys Town New York, Inc. is exempt from federal income taxes under Section 501c3 of the Internal Revenue Code. Boys Town New York, Inc. accounts for uncertainties in accounting for income tax assets and liabilities by recognizing the effect of income tax positions only if those positions are more likely than not of being sustained. At December 31, 2014 and 2013, Boys Town New York, Inc. had no uncertain tax positions accrued.

Amount consists of adjustments made to reflect expenses that were for special events 147,707. These were included as expenses for the audited financial statements but are a reduction to revenues for the Form 990.

Amount reflects value of items that were donated for silent auction. For audited financial statement purposes, only the cash proceeds were recorded. IRS990/Desc0Family Home Services consist of Family Homes for youth with serious behavioral or emotional problems who live in a family style home and receive care from professionally trained married couples called Family Teachers. Six to eight boys or girls, usually ages 12-18, live in each home. The Family Teachers along with two full-time Assistant Family Teachers are responsible for providing structured supervision for youth in their daily living and activities. This includes meeting the educational, medical, psychological, emotional, and behavioral needs of the youth in care. A major focus of this program is teaching youth social, independent-living, and educational skills, and helping them build healthy relationships with others. With a focus on reunification, the Family Teaching staff works with families to achieve permanency, safety, and child well-being. Like all services and programs in Boys Town New York, Inc.s Integrated Continuum of Care, treatment provided in Family Homes is based on the research-proven Boys Town Model.

The goal of this short-term residential program is to stabilize youth behaviors so youth can safely return home or to their designated placement. Youth work on established treatment plans that address problem areas. Specially trained staff members conduct behavioral assessments and provide referral recommendations and linkages to needed services. Boys Town New York, Inc. strives to provide safety and permanency through short-term services at three Intervention and Assessment Centers located in Brooklyn and Queens.

The main goal of In-Home Family Services is to prevent children from being placed outside of the home and/or to reunify them with their family if outside placement is necessary. Like all other services and programs in Boys Town New York, Inc.s Integrated Continuum of Care, treatment provided through In-Home Family Services is based on the research-proven Boys Town Model. Boys Town New York, Inc.s In-Home Family Services provide a family centered, skill-based intervention for families who are in or near crisis situation, and are at risk for having a child removed from the home. Treatment duration ranges from short-term to long-term, and service intensity ranges from highly intensive to less intensive, depending on a familys needs. Boys Town Family Consultants are available 24 hours a day, 7 days a week, and work with families in their homes to help caregivers build on their strengths, improve their parenting skills, and to identify community resources and supports.
