Form 990, Part III, Line 4D: Other Program Services Description

OTHER PROGRAM SERVICES 4: SEMP (OPWDD)SEMP provides enhanced supported employment supports for three individuals with developmental disabilities. Measurable outcomes include: 1) development of job-specific job-readiness skills; 2) travel training, and; 3) job placements upon completion of the 90-day job development period. City Access provided services to 26 units for the year ended June 30, 2016. OTHER PROGRAM SERVICES 5: Lavelle Fund for the BlindA Lavelle Fund for the Blind grant supports The Career Discovery Project (CDP), a multi-year project designed to annually provide appropriately supported, extended internship opportunities for blind or low vision high school students. CDP will be administered by CANY in collaboration with Commission for the Blind and Visually Handicapped (CBVH), and with the Metropolitan Museum of Art, the Queens Museum of Art, and the New York Hall of Science. CDP participants will: 1) develop core vocational skills as they explore new and motivating work options that are beyond their comprehension; 2) extend independent travel skills through CBVH-led travel training to internship sites; and 3) expand their social horizons through sustained involvement in new community settings.

IRS990/ProgSrvcAccomActy2Grp/Desc0Respite Programs (OPWDD)Respite Programs (OPWDD) Holiday provided hours of school holiday respite during the winter, mid-winter and spring recesses school year, for D75 students with developmental disabilities ages 14-21 and D75 Aspberger students, ages 14-19. A total of 28 students were serviced, 16 from Brooklyn and 12 from Staten Island. The two five-day holidays occur in February and April; placed a state licensed speech therapist with students during outings, to reinforce real-world communication skills. Contact hours were 8:30am-4:30pm each business day.Respite Summer provided hours of summer camp respite during August recess each school year, for 15 D75 students with developmental disabilities ages 14-21. Contact hours are 8:30am - 4:30pm each business day. Respite Weekend provided approximately 1,831 hours of weekend respite throughout the year for 100 D75 students with developmental disabilities ages 14-21. Contact hours are 8:30am - 4:30pm each business day.

CDP will be administered by CANY in collaboration with Commission for the Blind and Visually Handicapped (CBVH), and with the Metropolitan Museum of Art, the Queens Museum of Art, and the New York Hall of Science. CDP participants will: 1) develop core vocational skills as they explore new and motivating work options that are beyond their comprehension; 2) extend independent travel skills through CBVH-led travel training to internship sites; and 3) expand their social horizons through sustained involvement in new community settings.
