Mission

Making a difference in the lives of people; creating a healthy community

Partnership community health center, inc. (the "health center") was established and became operational in 2005 to provide quality, low-cost health care to the underserved and uninsured people of outagamie, winnebago, and calumet counties in wisconsin. The health center is primarily supported by private and third-party payments for medical services, as well as grants from governmental and other private not-for-profit organizations.

Monique dumond of community link consulting served as our cfo in 2024 and was compensated by community link consulting. Community link consulting was paid $245,528.67. Brian burmeister served as our pt chief strategy and information officer in 2024 and was compensated through his business of new healthcare solutions. New healthcare solutions was paid $87,700.00.

Form 990, Part VI, Section B, Line 12C

Conflict of interest policy applies to all members of the board of directors of partnership community health center, inc. (pchc) and its committees. A conflict of interest exists with respect to a matter for discussion or action by the board, any circumstance under which an interested person, by virtue of financial interest, may be influenced, or may appear to be influenced, either in whole or in part by any purpose or motive other than the success and well-being of the pchc and the achievement of its public charitable purposes. If, at any time, an interested person becomes aware that the board may or shall discuss or act upon any transaction or arrangement which may have any bearing of any kind upon, or may relate in any manner to, a financial interest of the interested person, such interested person shall disclose such financial interest to the board as follows: (a) the interested person shall provide to the board, in advance of such discussion or action by the board, written disclosure of the existence, nature and extent of the interested person's financial interest, or (b) if written disclosure cannot be provided in advance, e.g., in situations in which the interested person does not realize the nature of the transaction or arrangement to be discussed or acted upon until discussions have already begun, the interested person shall orally inform the board immediately upon such interested person's realization that the transaction or arrangement may bear upon or relate to a financial interest of the interested person, and shall provide such written disclosure to the board at the soonest practicable time thereafter. Any and all written or oral disclosures of financial interests shall be made a formal part of the minutes of the board. Notwithstanding the foregoing, an interested person shall not be required to disclose pursuant to this article 2.1 any financial interest that, in the exercise of such interested person's reasonable judgment, is so de minimis that it would not under any circumstances influence, or appear to influence, the interested person's judgment or actions with respect to the proposed transaction or arrangement to be discussed and/or acted upon by the board. Determination of conflict of interest: where an interested person has provided advance written disclosure of a financial interest but has not voluntarily recused himself or herself from discussion of or action upon the proposed transaction or arrangement, the remaining board members present shall, prior to commencing its discussion or taking action, determine by a majority vote whether the financial interest creates a conflict of interest, as defined above. The interested person shall not participate in any discussions or vote related to this determination, except to the extent necessary to fully explain the financial interest and the manner in which the proposed transaction or arrangement to be discussed or acted upon by the board may or will bear upon or relate to the financial interest. Procedures upon determination of a conflict of interest: in circumstances where the board has determined that a conflict of interest exists, the interested person shall not participate in any discussion or vote regarding the transaction or arrangement at issue, and shall not be present in the meeting room for any part of the discussion or vote relating to the transaction or arrangement. With respect to any transaction or arrangement with regard to which the board has determined that a conflict of interest exists, the board shall discuss such transaction or arrangement as appropriate, but shall not formally approve such transaction or arrangement unless and until the non-interested members of the board have decided, by 2/3 vote, that the transaction or arrangement is in the best interests of and for the benefit of the pchc, and is fair and reasonable thereto in all respects. In order to account for any uncertain tax positions, the health center determines whether it is more likely than not that a tax position will be sustained on examination by the taxing authorities based on the technical merits of the position, assuming the taxing authority has full knowledge of all information. If the tax position does not meet the more-likely-than-not recognition threshold, the benefit of the tax position is not recognized in the financial statements. The health center has not recorded any amounts due to uncertain tax positions.

IRS990/Desc0PARTNERSHIP COMMUNITY HEALTH CENTER (PCHC) IS A COMMUNITY-INITIATED, NON-PROFIT ORGANIZATION FORMED IN 1997 AS A "FREE CLINIC" TO PROVIDE NO COST MEDICAL CARE TO UNINSURED PATIENTS. IN 2004, PCHC WAS FUNDED AS A FEDERALLY QUALIFIED HEALTH CENTER AND HEALTH CARE FOR THE HOMELESS PROGRAM UNDER HRSA SECTION 330 FUNDING. NOW HEALTH CENTER JOINS NINETEEN OTHER COMMUNITY HEALTH CENTERS IN THE STATE OF WISCONSIN AND ALMOST 2,000 CENTERS NATIONWIDE.WE OFFER PRIMARY CARE, DENTAL CARE, AND INTEGRATED BEHAVIORAL HEALTH SERVICES ONSITE AT OUR FACILITIES IN GRAND CHUTE, WI, PRIMARY DENTAL SERVICES IN OSHKOSH AND WAUPACA WI, PCHC'S SERVICE AREA IS THE TRI-COUNTY AREA OF CALUMET, OUTAGAMIE AND WINNEBAGO COUNTIES AND ARE A FEDERALLY DESIGNATED MEDICALLY UNDERSERVED AREA. THE MIX BETWEEN URBAN, SUBURBAN, AND RURAL AREAS CREATES MANY UNIQUE BARRIERS TO CARE AND DISTINCT CHALLENGES TO OUR PATIENT POPULATIONS. THIS PAST YEAR 17,515 LOW-INCOME, HIGH RISK PATIENTS RECEIVED CARE AT PCHC. WE CONTINUE TO GROW TO MEET THE HEALTH NEEDS OF THE AREA'S UNDERSERVED AND IMPOVERISHED. OUR BOARD OF DIRECTORS' PRIORITY IS IMPROVING HEALTH ACCESS TO CARE FOR OUR PATIENTS AND STRATEGICALLY ENROLLING ALL ELIGIBLE PATIENTS IN MEDICAID AND NEW MARKETPLACE INSURANCE PRODUCTS.IN OUR SERVICE AREA THERE ARE IN EXCESS OF 100,000 PEOPLE LIVING AT OR BELOW 200% OF THE FEDERAL POVERTY LEVEL (FPL). CENSUS DATA INDICATES THAT POVERTY RATES CONTINUE TO RISE IN SINGLE PARENT HOUSEHOLDS, WITH INCREASES IN RENTAL PROPERTIES AND SUBSIDED HOUSING. LOW INCOME FAMILIES ON FREE AND REDUCED LUNCH IN AREA SCHOOL DISTRICTS ARE AT RECORD LEVELS AND NEW MINORITIES AND REFUGEES CONTINUE TO SETTLE IN THE AREA. THE DEMOGRAPHICS HAVE CHANGED DRAMATICALLY SINCE WE OPENED AS A SMALL HEALTH CENTER WITH A FOCUS ON HOMELESS, DISENFRANCHISED, AND MOSTLY UNINSURED ADULTS. WE HAVE NOW GROWN TO PROVIDE CARE TO A LARGE POPULATION CONSISTING OF MEDICALLY UNDERSERVED MULTICULTURAL FAMILIES, CHILDREN, ADULTS, ELDERLY, AND THE NEWLY INSURED. MANY OF THESE INDIVIDUALS SUFFER FROM CHRONIC MEDICAL CONDITIONS SUCH AS DIABETES, OBESITY, HYPERTENSION, AND OTHER CO-MORBID CONDITIONS DUE TO THEIR HISTORICAL LACK OF ACCESS TO HEALTH CARE AS AN EXTENSION OF POVERTY, NO INSURANCE OR HIGH DEDUCTIBLE INSURANCE, AND NO INCOME AVAILABLE TO COVER THE COSTS.DATA SHOWS THAT OUR PRACTICE CONTINUES TO RESPOND TO COMMUNITY NEED.. NEW DENTAL STAFF HAVE BEEN HIRED, AND NEW LOCATIONS HAVE BEEN OPENED, TO RESPOND TO THE AREA'S NEED. 93.3% OF PCHC PATIENTS HAVE INCOMES 200% OR BELOW POVERTY AND 71% LIVES AT OR BELOW THE POVERTY LEVEL. PCHC PROVIDES THE NECESSARY PRIMARY CARE AND PREVENTIVE SERVICES TO INDIVIDUALS THAT OTHERWISE WOULD NOT HAVE ACCESS TO CARE. WE HAVE SEEN A RAPID GROWTH IN NEW IMMIGRANTS FROM MEXICO, CENTRAL AMERICA AND ASIA, RECENTLY RESETTLED REFUGEES FROM THE CONGO, IRAQ, AFGHANISTAN, MIRAMAR, CUBA, AS WELL AS MANY NEW PATIENTS WITH COMPLEX MEDICAL AND BEHAVIORAL HEALTH ISSUES THAT REQUIRE CARE MANAGEMENT AND SOCIAL SUPPORT. PCHC IS THE ONLY FQHC IN THE AREA AND WE PARTNER WITH OUR LOCAL HEALTH SYSTEMS AND COUNTY HEALTH DEPARTMENTS FOR MANY SERVICES AND ARE THE PRIMARY SAFETY NET PROVIDER THAT OFFERS A SLIDING FEE AND CARE FOR THE UNINSURED WITHIN OUR TRI-COUNTY SERVICE DELIVERY AREA.
