“In august 2024, the local became aware it was the victim of a criminal email phishing scheme. As a result of the scheme, a total of $1,945,000 was fraudulently misappropriated from the local. In an effort to recover the funds, the local immediately notified the appropriate law enforcement authorities as well as its banks. As of the filing date of this 990, and as a result of the local's actions, a total of $693,613 was recovered in 2024, as well as $92,923 in 2025. The local also contracted with an independent accounting firm to investigate the fraudulent transactions and implemented improved safeguards to prevent against any future attacks.”

“The accounting firm forwarded the form 990 to the director of finance. The director of finance received legal review and provided the form 990 to union officers for review. The form 990 was signed by either the president or vice president. The executive board was informed after filing.”

Accounting would report to the uhw chief of staff, president and/or vice president any apparent conflict of interest found. In a confidential manner, the outside ethics officers and the in-house ombudsperson investigate/process any complaint about conflicts of interest submitted by any members or staff. The policy provides protection against retaliation and ensure confidentiality of the reports.”
