Form 990, Part VI, Section B, Line 11B

Describe the process to review this form 990 at the end of the fiscal year, the profit and loss (p & l) statement, general ledger & balance sheets are generated for july 1st through june 30th. The documents (p & l, g/l & balance sheet) are forwarded to the api sbp program manager for review and analyzing. The data is analyzed and used to fill out the form 990, schedule a, schedule b, schedule i, schedule o and schedule r accordingly. The forms are then forwarded to the accounting manager in the ltsc cdc fiscal department for review and final approval. Once the forms are approved by the accounting manager, the forms are forwarded to an authorized signatory for api sbp, inc. Once signed, copies are made for internal files and the originals are filed directly with the irs.

The organization requires all directors to annually sign a statement affirming a) receipt of the organization's conflict of interest policy, b) understanding of the policy, and c) agreement with the policy. The organization's conflict of interest policy describes how the organization will resolve possible conflicts of interest.
