Activity 2

Healthy Moms Healthy Babies is an outreach program to identify pregnant women and get them into care, and enhancement of home visiting programs through training and other infrastructure support. From July 1, 2012 through June 30, 2013, information was shared with 1,262 consumers, of whom 111 were pregnant women.The program also generated $229,532 in government grant contributions included on Form 990, Page 9, Part VIII, line 1(e).

Activity 3

Perinatal Network services - The Network has provided perinatal education and information to medical, health and human service providers through educational workshops and conferences, and to consumers through consumer workshops, and to hundreds of community members through attendance at health fairs and other community events. In 2012-2013, 354 providers attended CPPSN-sponsored events and 1,139 consumers attended health education workshops.The program also generated $184,801 in government grant contributions included on Form 990, Page 9, Part VIII, line 1(e).
