ALL OF THE LATTER PROGRAMS ARE COMBINED WITH OTHER ASSOCIATION PROGRAMS TO ASSIST ELDERLY INDIVIDUALS BY 1) PREVENTING THE ONSET OF PROBLEMS, 2) MAINTAINING INDIVIDUALS AT THEIR PRESENT LEVEL OF LIFE ACTIVITY, OR 3) IMPROVING THEIR LEVEL OF LIFE ACTIVITY.

AN OMBUDSMAN IS AN ADVOCATE FOR PATIENTS AND THEIR FAMILIES. EACH OMBUDSMAN VISITS AN ASSIGNED FACILITY WEEKLY TO VISIT RESIDENTS, DISCUSS PROBLEMS WITH STAFF, AND TALK WITH FAMILIES. IF PROBLEMS ARE IDENTIFIED, THE OMBUDSMAN IS THE FACILITATOR FOR ALL INVOLVED INDIVIDUALS. IN SEVERE SITUATIONS, THE OMBUDSMAN REFERS ALL SITUATIONS TO A STATE REGULATORY AGENCY FOR OFFICIAL INVESTIGATION. THE PEER SPECIALIST PROGRAM PROVIDES SERVICES TO NIAGARA COUNTY'S SEVERELY AND PERSISTENTLY MENTALLY ILL ADULTS. THE SERVICES OFFERED BY THE PROGRAM INCLUDE EDUCATION THAT WILL ASSIST RECIPIENTS IN BUILDING LIFE SKILLS, LINKS TO SERVICES SUCH AS EDUCATIONAL AND EMPLOYMENT OPPORTUNITIES, ASSISTANCE WITH LOCATING SAFE, AFFORDABLE, INDEPENDENT HOUSING, AND FACILITATING THE INVOLVEMENT OF THE CLIENT IN THEIR TREATMENT PLAN.THE VISUALLY AND HEARING IMPAIRED PROGRAM IS TAILORED TO MEET THE INDIVIDUAL NEEDS OF THOSE WITH SENSORY LOSS. SERVICES TO THE VISUALLY IMPAIRED AND BLIND INCLUDE HOME MANAGEMENT COMMUNICATION SKILLS TRAINING, SENSORY AWARENESS, COUNSELING AND ASSISTANCE IN ADJUSTING TO SIGHT LOSS, AND REFERRALS TO PHYSICIANS AND OPHTHALMOLOGISTS. SERVICES TO THE HEARING IMPAIRED AND DEAF INCLUDES INTRODUCTION TO SPEECH AND LIP READING, COUNSELING AND ASSISTANCE IN ADJUSTING TO HEARING LOSS, AMPLIFICATION AIDS AND DEVICES, AND REFERRALS TO HEARING SERVICES AND AUDIOLOGISTS. THE GERIATRIC NURSE PROGRAM ENABLES PEOPLE TO STAY IN THEIR OWN HOMES AND PREVENT FUTURE HOSPITALIZATION. ONE NURSE WHO ASSESSES CLIENT NEEDS AND SERVES AS A REFERRAL SOURCE TO ESTABLISHED COMMUNITY SERVICES SERVES THE GERIATRIC POPULATION.THE CALL AND REASSURE ELDERS (CARE) PROGRAM IS A TELEPHONE REASSURANCE PROGRAM INTENDED TO SERVE AREA SENIORS OR DISABLED ADULTS WHO ARE HOMEBOUND, ISOLATED, LIVING ALONE OR IN NEED OF DAILY CONTACT TO ENSURE PERSONAL SAFETY. A TRAINED VOLUNTEER WILL PLACE A PHONE CALL ON A REGULARLY SCHEDULED BASIS TO SENIORS WHO ARE IN NEED OF SOMEONE TO TOUCH BASE WITH THEM TO ENSURE THEIR WELL-BEING AND/OR TO HAVE A FRIENDLY CONVERSATION.THE MEMORY MINDERS PROGRAM IS A SOCIAL DAY PROGRAM FOR PEOPLE WITH EARLY MEMORY IMPAIRMENT. PARTICIPANTS ENJOY ACTIVITIES AND SUPPORT SUCH AS ART, MUSIC, GAMES, EXERCISE, OUTINGS AND SOCIALIZATION WHICH ARE DESIGNED TO SLOW THE FURTHER DETERIORATION OF MILD MEMORY LOSS. TECHNIQUES ON HOW TO DEAL WITH THE PROBLEMS RELATED TO MEMORY LOSS ARE TAUGHT IN THE PROCESS.THE COMMUNITY-BASED CARE TRANSITIONS PROGRAM IS A PARTNERSHIP BETWEEN THE ASSOCIATION AND P2 COLLABORATIVE OF WNY WITH OVERSIGHT BY THE CENTERS FOR MEDICARE AND MEDICAID SERVICES. THE GOAL OF THE PROGRAM IS TO IMPROVE THE HEALTH OF AGED INDIVIDUALS IN NIAGARA COUNTY WHO ARE TRANSITIONING FROM THE HOSPITAL BACK INTO THEIR HOMES AND TO REDUCE HOSPITAL READMISSIONS. PARTICIPATION IN THE PROGRAM IS COMPLETELY VOLUNTARY AND DESIGNED TO HELP PATIENTS AND THEIR LOVED ONES BETTER UNDERSTAND AND MANAGE THEIR MEDICATIONS, PREVENT RETURN VISITS TO THE HOSPITAL BY IDENTIFYING EARLY WARNING SIGNS THAT ALERT THE PATIENT THAT ADDITIONAL MEDICAL CARE MAY BE NEEDED, AND BE AN ACTIVE PARTICIPANT IN FOLLOW-UP VISITS WITH THEIR PRIMARY CARE PROVIDER.
