The audit committee meets with the external auditors to review the draft of the form 990. Once the draft of the form 990 has been approved by the audit committee, the board of directors reviews and approves the draft form 990 prior to filing with the irs.

The organization regularly and consistently monitors and enforces compliance with the conflict of interest policy. On at least an annual basis, the policy is reviewed and approved by the board of directors. In addition, each member of the board is required to complete an annual conflict of interest disclosure form which is then reviewed and approved by the board of directors.

Residents' money is held for them to use for personal benefit.

PART V, LINE 4:

The association's intended use of its endowment funds is to further the exempt purpose of the organization. In addition, the association has a net interest in the endowment funds of the e.j.g. Foundation, inc., (supporting organization). A separate board of directors controls the funds of the foundation. The association has no authority to access these funds. The foundation administers the association's interest in the endowment fund, whose principal is permanently restricted and interest earned is distributed to the association for capital purposes.

PART X, LINE 2:

The association has been determined to be exempt from federal income taxes under section 501(c)(3) of the internal revenue code and have been classified as an organization that is not a private foundation under section 509(a). Management is unaware of any unrelated business activities that may be subject to unrelated business income tax or any activities that would jeopardize the associations exempt status.

THE DAY HABILITATION PROGRAM PLAN IS DESIGNED SPECIFICALLY TO MEET EACH INDIVIDUAL'S NEEDS, WHILE PROVIDING MEANINGFUL AND PRODUCTIVE ACTIVITIES BASED ON THEIR VALUED OUTCOMES IN ORDER TO ACHIEVE AND/OR RETAIN THE GOALS THEY DESIRE IN LIFE. THIS IS A MEDICAID FUNDED PROGRAM OPERATED UNDER OPWDD. RESIDENTIAL HABILITATION: SERVICES HELP INDIVIDUALS WITH DAILY LIFE ACTIVITIES AND ARE USUALLY PROVIDED WITHIN THE HOME ENVIRONMENT. THE UNIVERSAL PRE-K PROGRAM WORKS WITH AREA SCHOOL DISTRICTS TO HELP PROVIDE PRESCHOOL SERVICES FOR FOUR YEAR OLDS. COMMUNITY PRESCHOOL IS AVAILABLE FOR A NOMINAL FEE FOR THREE TO FIVE YEAR OLDS. THE CURRICULUM FOR THE INTEGRATED PRESCHOOL CLASSROOM IS BASED ON DEVELOPMENTALLY APPROPRIATE PRACTICES WITH AN EMPHASIS ON CHILD-CENTERED ACTIVITIES AND IS ALIGNED WITH THE N.Y.S. STANDARDS. CLASSES ARE HALF-DAY (2 1/2 HOURS) AND MAY HAVE UP TO 15 STUDENTS. THERAPY SERVICES: SPEECH/LANGUAGE, PHYSICAL AND OCCUPATIONAL THERAPY CAN BE PROVIDED TO AREA SCHOOL DISTRICTS ON A CONTRACT BASIS AND IS AVAILABLE TO FAMILIES PRIVATELY. IN ADDITION, THE E.
