“FORM 990 - PART I, LINE 1 AND PART III, LINE 1: FINDING THE CURE FOR PARKINSONS TAKES AN ORGANIZATION WITH EXTRAORDINARY VISION. Actor Michael J. Fox established The Michael J. Fox Foundation for Parkinson's Research (the "Foundation"), incorporated in Delaware in 2000, after publicly disclosing in 1998 that he had been diagnosed with Parkinson's disease seven years earlier, at age 29. Today, The Michael J. Fox Foundation is the world's largest private funder of Parkinson's research. It is dedicated to accelerating a cure for Parkinson's disease and improved therapies for the estimated five million people living with the condition today. The Foundation pursues its goals through an aggressively funded, highly targeted research program coupled with active global engagement of scientists, Parkinson's patients, business leaders, clinical trial participants, donors and volunteers. In addition to funding more than $700,000,000 in research through the end of December 31, 2016, the Foundation has fundamentally altered the trajectory of progress toward a cure. Operating at the hub of worldwide Parkinson's research, the Foundation forges (i) groundbreaking collaborations with industry leaders, academic scientists and government research funders; (ii) leverages new technologies to amplify the patient voice in Parkinson's research; (iii) mobilizes patients and loved ones to increase the flow of participants into clinical trials; and (iv) coordinates community engagement efforts including patient policy advocacy, education and community building through the grassroots involvement of thousands of Team Fox members around the world. “GOVERNANCE, MANAGEMENT AND DISCLOSURE: Line 2: BOARD MEMBER RELATIONSHIPS Two board members are in-laws and two sets of board members are married. Line 11A: Process for Review of Form 990: The Finance Committee of the Board of Directors, in association with external auditors, approves the annual IRS Form 990 which is then made available to the entire Board before it is filed. Line 12A: Conflict of Interest Policy monitoring: Officers, directors and key employees complete a conflict of interest questionnaire annually. The information is reviewed by the Chief Financial and Administrative Officer, and conflicts are reviewed with the Board of Directors. Line 15: PROCESS FOR DETERMINING COMPENSATION The Compensation Committee of the Board of Directors reviews and approves compensation of key employees annually.
