“OTHER PROGRAM SERVICES 4: Social Skills - (Sessions on Sundays). This unique program has been designed for individuals ages 5 and older. It is a curriculum comprised of individualized programs, using the most current research supporting the best practices to teach social skills through the use of positive reinforcement and practice, during parallel and group activities. A total of 26 children participated in the agency's Social Skills program with a total of 43 sessions for the year ended June 30, 2024. OTHER PROGRAM SERVICES 5: Day Habilitation Program - The Day Habilitation program participants engage in services based on their individualized needs, preferences and goals. Personalized activities and supports enable the participant to live, work, socialize and participate in the community. A total of 39 individuals participated in the agency's Day Hab program with a total of 7,411 service days for the year ended June 30, 2024. OTHER PROGRAM SERVICES 6: Art History - presents a different artist each week, allowing participants to create their own interpretation of specific artistic genres. Participants will be able to learn about an artist while exploring and discovering their own talents. The Art History had 20 individuals participate with a total of 47 sessions for the year ended June 30, 2024. OTHER PROGRAM SERVICES 7: InHome Respite - In-Home Respite - A service, provided by trained staff, providing relief from care giving to families of individuals with a developmental disability, offering support and relief to the primary caregivers. A total of 9 individuals participated during the year with 6,690 units provided for the year ended June 30, 2024. OTHER PROGRAM SERVICES 8: The Gym 2 Program (PS69) is designed to address Physical Strengthening, Increased Mobility, Advanced Social Skills, and Teamwork. Individuals participate in Stretching, Arm Exercises, Ball Play: (Catch, Basketball, Soccer, Pitching, etc.), along with Aerobic Movement to Music. A total of 5 individuals participated during the year with 20 sessions provided for the year ended June 30, 2024. OTHER PROGRAM SERVICES 9: Community Rehabilitation - Community Habilitation is a Medicaid-funded program operated under OPWDD to provide one-to-one training to people with intellectual/developmental disabilities to develop or enhance the skills needed to live more independently in their homes or in the community.A total of 16 individuals participated during the year with 3,252 program hours provided for the year ended June 30, 2024. OTHER PROGRAM SERVICES 10: Art and Artists - available to all age groups...this program relates to animiation, illustration, and cartooning. There are also discussions about artists in these artistic fields. The Art and Artists program had 8 individuals participate with a total of 50 sessions for the year ended June 30, 2024.”

“GRACE entered into a license agreement in November 2020 with New York City Health and Hospitals Corporation to occupy space at the Sea View Rehabilitation Center (Sea View) to operate support programs for individuals with autism spectrum disorders for a period of five years which expires on August 31, 2025, with an option for an additional five years including rent and utilities. The space, which consists of approximately 6,900 square feet, was donated to allow GRACE to expand its community services. GRACE has utilized the facility space at Sea View sinceMarch 1, 2015.The value of the donated rent and utilities was $210,174 for the years endedJune 30, 2024 and 2023, respectively, which is reported as In-Kind Donation on the statements of activities and as Occupancy on the statements of functional expenses.”
