In addition, upon request of any board member, a copy of the form 990 will be provided. If there are any material changes noted thereon, an amended form 990 will be filed.

The organization enforces the conflict of interest policy for board members and key employees by monitoring known relationships and by requesting the completion of annual conflict of interest questionnaires from each board member and key employee. All responses are reviewed annually by the audit committee to determine if any reported items represent a conflict of intertest and, if so, to determine whether such transactions are permissible and in the organization's best interest. If any such conflict involves a member of the audit committee, such individual is recused from the determination of whether or not such reported item does represent a conflict as well as whether or not this transaction is permissible and in the organization's best interest.
