Form 990, Part VI, Line 11B Review of form 990 by governing body

The form 990 was prepared by the montefiore's tax department working closely with white plains' finance team and assisted by various departments throughout the health system. The form 990 was reviewed and approved by montefiore vice president-finance and the white plains hospital senior leadership including the chief financial officer. In addition, an independent accounting firm was engaged to review the form 990. Upon completion of the various reviews, the form 990 was presented to the white plains audit/compliance and executive compensation committee of the board of trustees for review and approval. Once approved by the audit/compliance and executive compensation committee of the board of trustees, the form 990 was provided to all members of white plains' governing body prior to filing.

Form 990, Part VI, Line 12C Conflict of interest policy

The foundation has adopted the conflict of interest policy maintained by white plains hospital medical center, the parent organization. All monitoring and enforcement of the conflict of interest policy is performed by the hospital on behalf of the foundation. All officers, directors and key employees of white plains are required to complete an annual conflict of interest questionnaire, in their capacity as an employee of the hospital or as a board member. Completed questionnaires are reviewed by the legal committee of the board of directors and concerns presented by the responses to the conflict of interest policy are disclosed to the board, with the interested party recused from discussing the matter.
