Form 990, Part VI, Line 11B: Form 990 Review Process

Upon preparation of Form 990, the FCI Director for Finance and Administration reviews all schedules. The president and the members of the Audit and Finance Committee also review the return prior to filling with IRS.

Form 990, Part VI, Line 12C: Explanation of Monitoring and Enforcement of Conflicts

A conflict of interest procedures framework for identification, disclosure, management, and monitoring of real or perceived conflict of interest has been developed and issued under the Human Resources Policy Manual.FCI's Human Resource and Finance Department jointly monitor and enforce staff compliance with the organization's Conflict of Interest policy. FCI's By-lawsguide conflict of interest to the Board of Directors, requiring a Director to disclose any possible conflict of interest to the Board and charging the Board with deciding whether said members should resign, take a leave or alternatively, recuse himself/herself in matters where a possible conflict of interest, real or perceived, exists.

Form 990, Part VI, Line 19: Other Organization Documents Publicly Available

FCI's audited financial statements and Form 990 are available at www.familycareintl.org. Annually, the organization sends its audited financial statements to institutional donors and summary information from the audited financial statements to select individual donors. Additionally, financial information, governing documents and conflict of interest policy are provided upon request.

Part X : FIN48 Footnote

Generally accepted accounting principles require management to evaluate tax positions taken by FCI and recognize a tax liability or an asset if FCI has taken an uncertain tax position that more likely than not would not be sustained upon examination by the applicable authorities. Management believes there are no material uncertain tax position that require recognition in the accompanying financial statements.FCI is subject to routine audits by taxing jurisdictions; however, there are currently no audits for any tax periods in progress.
