Additional Explanations

FORM 990, PART XI, LINE 9:

Gain on interest rate swap contract 6,357.

FORM 990, PART XII, LINE 2C:

The organization has a committee to oversee the financial statement audit and the selection of the independent auditor. This process has not changed from prior year.

Financial Statement Notes

PART X, LINE 2:

The agency is exempt from federal income taxes as a not-for-profit corporation under tax section 501(c)(3) as determined by the internal revenue code. In addition, the agency qualifies for charitable contribution deductions and has been classified as an organization other than a private foundation. Under accounting standards codification (asc) section 740, the tax status of tax-exempt entities is an uncertain tax position, since events could potentially occur that jeopardize tax-exempt status. Agency management is not aware of any events that could jeopardize tax-exempt status. Therefore, no liability or provision for income tax has been reflected in the financial statements. Management believes the agency is no longer subject to review by the internal revenue service for tax years prior to 2011
