Mission

THE MISSION OF CEREBRAL PALSY ASSOCIATIONS OF NEW YORK STATE IS TO ADVOCATE AND PROVIDE DIRECT SERVICES WITH AND FOR INDIVIDUALS WITH CEREBRAL PALSY AND OTHER SIGNIFICANT DISABILITIES, AND THEIR FAMILIES, THROUGHOUT NEW YORK STATE IN ORDER TO PROMOTE LIFELONG OPPORTUNITIES AND CHOICES FOR INDEPENDENCE, INCLUSION AND ENHANCED QUALITY OF LIFE. In January 2019, United Cerebral Associations of New York State, Inc. changed its d/b/a from Cerebral Palsy Associations of New York State to Constructive Partnerships Unlimited. As of January 2019, the updated mission of United Cerebral Palsy Associations of New York State, Inc. is to provide opportunities for people to achieve fulfilling lives.

FORM 990, PART I, LINE 1 (CONTINUATION):

Disabilities in need of support. The agency offers several programs to promote opportunities for people with intellectual and developmental disabilities to achieve fulfilling lives through community living, day programs, vocational and employment programs, medical and clinical services as well as early intervention, recreational and family support services.

FORM 990, PART III, LINE 4A (CONTINUATION):

Community habilitation - cconstructive partnerships unlimited provides community habilitation and individualized support service (iss) for 120 individuals with intellectual disabilities. The focus is to maintain or develop the highest level of independence possible for each person. The service is based on the individual's needs and is geared to support their independence. Services are provided in all five boroughs of new york city, 365 days a year. The services are goal-oriented with measurable outcomes and person-centered. Deliverable services can include, but are not limited to, community safety awareness, activities of daily living, travel, training, money management, and community integrations. Medicaid waiver from opwdd for people with disabilities. The service can be provided in the individuals home, in the community, and in the independent residential alternative (ira). The service is delivered by a trained individual living skill trainer ("ilist"). The individual and staff work together to develop goals that will assist the individual to live more independently in their desired setting. Iss housing is a grant that is funded through opwdd. The individual can live independently or with a roommate. Currently, cp unlimited has 62 individuals residing in iss housing. In most cases, constructive partnerships unlimited provides community habilitation services to support the individual moving into an apartment for the first time. Wrap-around services that require the services of a health aide are also available. Nys emergency shelter - contstructive partnerships unlimited provides temporary housing of up to 55 units. This program is currently occupied by 33 families totaling 117 individuals who are in need of emergency housing. The emergency shelter program provides 24 hour, 7 days a week support including food and clothing services, transportation, childcare, job training and placement, and housing and recreation services. Program expenses: $183,300,790. Program revenue: $193,480,814.

Group day habilitation with walls (site based) - our site based day habilitation programs provide diverse opportunities to enhance their independent living and vocational skills. Agency transportation is provided to all individuals who attend these programs. A program nurse is available at all locations to attend to a person's medical needs. Our site based programs are also attached to our article 16 clinics and provide physical therapy, speech therapy and occupational therapy to our participants. All our day and vocational programs are actively enrolling new individuals. The agency has five site based group day hab locations throughout new york city, with a capacity to service 594 individuals. Group supplemental day habilitation without walls - our supplemental day habilitation program provides recreational and social opportunities for people diagnosed with developmental disabilities after 3 pm (monday to friday). Agency transportation is provided to all individuals who attend this program. Group day habilitation without walls and communtiy prevocational services - day habilitation without walls and community prevocational services are fully community-based person centered programs utilizing multiple volunteer sites in nyc area to provide our people with meaningful and productive activities. The voluntary sites are located all over nyc and the sites are allocated based on each person's vocational interest. Individuals best suited for this program are capable of using access-a-ride or public transportation to commute to volunteer site. Limited agency transportation is available for this program. Presently, there are 102 individuals actively paticipating. Supported employment (semp) - supported employment provides ongoing support for individuals to obtain and maintain competitive employment. Job coaches assist the individuals with job search, interviews and 1:1 job coaching. Semp services are supported by access-vr funding or by nys opwdd. There are currently 25 individuals participating. Access-vr is a program that provides job coaching, career counseling and job development programs funded by doe. There are currently 9 individuals participating. Program expenses: $35,535,677. Program revenue: $41,807,926.

The rehab department operates an article 16 clinic with enrollment of over 718 of patients. This clinic supports our patients through direct occupational, physical and speech therapy services with emphasis in restoring or maintaining their functional abilities. The clinic is overseen by a psychiatrist who also evaluates patients for complex medical equipment such as custom wheelchairs, seating units, and bathing devices. These devices become an integral part of a patients life as it offers them the opportunity to engage in daily activities. Lastly, the article, support individuals who require counseling/psychotherapy services to address behavioral and emotional issues with the ultimate goal of enhancing an individuals quality of life and increasing their ability to pursue personal goals and choices. Program expenses: $4,551,882. Program revenue: $2,555,751.
