PSC Community services, Inc. has its Form 990 prepared by an outside accounting firm and has established the following review process to ensure that the information reported is complete and accurate. When the Form 990 has been prepared, reviewed by management and is ready to be filed with the Internal Revenue Service, it is electronically sent to the Board for approval/ The 990 is presented to the Board of Diectors at a meeting prior to filing. Once the Board has approved the return, it is filed with the Internal Revenue Service.

Form 990, Part VI, Section B, Line 12C

The conflict of interest policy covers all individuals such as an Officer, Director, Committee Membr, Supervisory Staff or any agency personnel serving in a leadership capacity. All conflicts of interest from voting or otherwise attempting to affect any decision for the agency to participate or not to participate in such transaction and the manner or terms of such participation. Annually, at the June monthly meeting of the Board of Directors, all members of the Board of Directors, Committee Members and Supervisory Staff shall disclsoe any direct or indirect relationships with organizations, either for profit or not for profit, that may, during their term of office or tenure, be involved with the agency in a formal capacity. Such disclosue shall be on the agency disclosure form.

The Executive Committee of the Board of Directors uses Guidestar.org to review salaries of Executive Directors of similar sized organization in the area. Then based on their performance, the Board of Directors, suggest and approve the compensation of the Executive Director. The approval is documented in the minutes to the Board.
