Form 990 Supplemental Information

FORM 990, PART VI-A, LINE 6 ORGANIZATION MEMBERS: The organization is organized as a not-for-profit corporation with a member, effective April 1st, 2017, which is known as NYU Langone Health System. FORM 990, PART VI-A, LINE 7A ELECTION OF GOVERNING BODY: NYU Lagone Health System is the sole member of the corporation. This member can approve the election of the Board of Directors of the corporation in accordance with the bylaws. FORM 990, PART VI-A, LINE 7B DECISIONS OF GOVERNING BODY SUBJECT TO APPROVAL: The governing body can approve and monitor the corporation's operating and capital expenditure plan on an annual basis provided, however, the operating and capital expenditure plan is reviewed and approved by the member to ensure that such budgets conform to the mission and philosophy of the corporation. FORM 990, PART VI-B, LINE 11A FORM 990 REVIEWED BEFORE FILING: The Form 990 is reviewed by a voting member of the organization's governing body, an officer (CFO) of the corporation, and tax consultant before it is filed with the IRS. The review is conducted independently by each person mentioned above. Questions, changes or recommendations are reported in writing or verbally. A copy of the Form 990 is provided to the organization's governing body before it is filed, together with an overview of relevant procedures (e.g. compensation, governance and due diligence, etc.) FORM 990, PART VI-B, LINE 12C COMPLIANCE WITH CONFLICT OF INTEREST POLICY: All Board Members and Key Employees are expected to read and understand the Conflict of Interest Policy which is mailed annually with the disclosure statement and is available in the Administrative Policy & Procedure Manual. Key Members include Senior Management and all Department Heads, Chairman of Departments, and other employees (including physicians) who are in a position to influence any substantive business decision between the Hospital and/or a Vendor. In addition, all other Hospital Personnel are trained and educated as to their obligations to report any potential Conflict of Interest. Training and Education are conducted for all employees upon hire and annually thereafter. Annually the Conflict of Interest Policy and Disclosure Statement are sent to all Key Members of NYU Winthrop Hospital. Key Members are reminded of their continuing obligation to update the annual Disclosure Statement whenever new information arises. If the Compliance Officer believes a potential or actual conflict of interest exists, the Compliance Officer will seek outside counsel's advice and prepares a report stating the relevant facts with an explanation as to why an actual or potential conflict of interest exists. The actual or potential conflict will be reported to the Audit and Corporate Compliance Committee of the Board for review. Once a final decision is made on how to proceed, the Personnel will be instructed as to the appropriate corrective action. FORM 990, PART VI-B, LINES 15A & 15B COMPENSATION REVIEW/APPROVAL: The organization board adopted a Compensation Policy (The "Policy") for covered individuals. Pursuant to the policy, a complensation committee of independent directors was establised to review the compensation of all employees specified as having a substantial influence over the organization and who receive remuneration from the organization, including, among others, the organization's President and Chief Executive Officer, the organization's Chief Operating Officer and the organization's Chief Finanical Officer. The compensation committee is advised by outside counsel and the consultant opines to the compensation comittee that the level of compensation paid and the process by which compensation is established meet applicable IRS reasonableness and "Safe Harbor" standards. The outside compensation consultant provides data of compensation provided at similar organizations to ensure that the organization does not compensate in excess of market norms. FORM 990, PART VI-B, LINE 19: Governing

Mission Statement

FORM 990, PART III, LINE 1: It is the mission of NYU Winthrop to provide high-quality, safe, culturally competent, and comprehensive healthcare services in a teaching and research environment, which improve the health and well-being of the residents of Nassau County and contiguous county areas, based on a profound commitment to an enduring guiding principle - "Your Health Means Everything". The Hospital's Community Service Plan is guided by and reflects its mission statement - to improve the health and well-being of the residents it serves. FORM 990, PART III LINE 3: On April 1, 2017 Winthrop-University Hospital (WUH) completed an affiliation agreement with NYU Langone Health System (NYUHS) and NYU Langone Hospitals, whereby the NYUHS became the sole corporate member of Winthrop-University Hospital. As a result of the acquisition, WUH changed its name to NYU Winthrop Hospital (NYU Winthrop). NYUHS is also the sole corporate member of NYU Langone Hospitals. The affiliation had two phases: in the first phase of the agreement NYU Winthrop Hospital (www.nyuwinthrop.org) remained a separate entity, responsible for its own liabilities and obligations. In the second phase of the affiliation, which occurred on August 1, 2019, Winthrop merged with and into NYU Langone Hospitals.

FORM 990, PART III, Line 4

COMMUNITY PARTNERSHIPS - NYU Winthrop remains engaged with the community by maintaining relationships with community- and faith-based organizations, private organizations, local school districts, and legislators from the village, town, county, state and federal government. Our partnerships have been strengthened through quarterly Community Cultural Advisory Committee meetings addressing the needs of the underserved, as well as increased efforts to reach out to the community to achieve goals and objectives identified through the Community Health Needs Assessment. On a broader scale, the hospital is also an active member of the Long Island Health Collaborative (LIHC), a coalition that includes the Nassau and Suffolk County health departments, all Long Island hospitals, health and social service community-based organizations, health plans, academic institutions, local government, and other groups that are working together toward a healthier Long Island. By using the collective impact model in Population Health efforts, it is hoped that public awareness and education will help motivate individuals to make the changes necessary to develop a healthy lifestyle and improve outcomes. NYU Winthrop hospital participated in the collaborative assessment that was done by the Long Island Population Health Improvement Program, the grant initiative of LIHC.

Health Initiatives Supporting the Nys Prevention Agenda

INTERVENTIONS: In June 2019 NYU Winthrop's Board of Directors approved its triennial Community Health Needs Assessment (CHNA). From this, a Community Service Plan was developed. Selection of targeted strategies is data driven, supported by research and discussion with community partners. Our primary focus areas include: 1) Prevention and Management of Chronic Disease, and 2) Promotion of Well-Being and prevention of Mental Health and Substance Abuse. Additionally, the Plan will include focus on 3) Promoting a Healthy and Safe Environment, 4) Birth-Related Disparities, 5) Improvement of Health Literacy, and 6) Healthy Foods The following explains initiatives taken to address the needs identified in NYU Winthrop's 2016 Community Health Needs assessment, many of which will continue in support of the updated CHNA: A) NATIONAL DIABETES PREVENTION PROGRAM - NYU Winthrop offered the evidence based National Diabetes Prevention Program at no cost at the Diabetes Education Center. Three classes began in 2017 and ended mid 2018:39 participants, 71% attendance rate, average weight loss 13 lbs. B) TAI CHI FOR ARTHRITIS - Targeting Chronic Disease, Stress Management & Fall Prevention NYU Winthrop offered the evidenced-based program Tai Chi for Arthritis & Fall Prevention Program at no cost. Medical studies have shown that Tai Chi relieves pain and improves the quality of life for people living with arthritis, though participants do not need to have arthritis to participate. The response to the program was tremendous, with community members calling to request classes. There were a total of 150 participants. Post satisfaction surveys indicate an increased feeling of wellness, improved energy, posture, and confidence. C) CHILDHOOD OBESITY SCREENINGS IN THE COMMUNITY - NYU Winthrop collaborates with the Head Start communities located in Hempstead and Westbury to bring the 5-2-1-0 healthy lifestyle program to their families and initiated this same program at its Hempstead Children's Health Clinic. The program encourages 5 servings of fruits and vegetables each day, no more than 2 hours of screen time, 1 hour of exercise and 0 sugary drinks. The interventional strategy includes documenting each child's BMI, giving participants a packet of the 5-2-1-0 guidelines, and offering nutritional counseling for families of children identified as having an unhealthy weight. Children were measured 10/17, 3/18, and 7/18. We captured the following statistics from the 99 children who attended all three sessions: 10/17 - 56.1% Normal, 18.35% High, and 24.45% Obese; 3/18 - 73.1% Normal, 17.8% High, and 17.8% Obese; 7/18 - 58.65% Normal, 23.35% High, and 23.5% Obese. Although we offered nutritional counseling in the form of a support group, parents did not respond. Head Start's nurse has been supportive of the program. Note: at the request of Head Start we expanded the program to Roosevelt and Freeport, two additional communities with health disparities. D) PROMOTE EXCLUSIVE BREASTFEEDING - Cited as a "Strategy to Prevent Obesity and Other Chronic Diseases" by the CDC, NYU Winthrop, designated "Baby Friendly" in 2014, educates new mothers about breastfeeding and provides supportive services to improve the percentage of newborns who only receive breast milk when discharged from the hospital. Our records show that,from Janaury 2017 through April 2019 an average of 41% of babies were only receiving breast milk when discharged from NYU Winthrop. To encourage this health initiative NYU Winthrop offers a free breastfeeding support group, led by a lactation consultant. E) INCREASE EFFORTS TO RAISE PARTICIPATION IN COLORECTAL CANCER & BREAST CANCER SCREENINGS - NYU Winthrop participated in the American Cancer Society's 80% by 2018 Colorectal Cancer Screening Initiative and in the NAPBC (National Accreditation Program for Breast Centers) Patient Navigation Initiative. Colorectal Cancer - Nurse Navigators contacted 18 community-based organizations to encourage participation

F) FALL PREVENTION PROGRAM - NYU Winthrop's Fall Prevention Program for seniors includes a beginner four-part fall prevention workshop, followed by exercise classes to improve balance, flexibility and strength. NYU Winthrop performs balance assessments for individuals interested in taking a fall prevention class. Those who are deemed unfit to take classes are advised to check with their physician or physical therapist. These services were provided free of charge.During fiscal year 2019, 11 four part series were held at the Winthrop Welcome Center with a total of 126 attendees; 2 four part series were held off-site, with 56 attendees. A total of 198 balance assessments were held. Forty five exercise classes were held at the Welcome Center with 400 participants. Twenty six exercise classes were held in the community, with 608 participants. Participants fill in post-satisfaction forms. They acknowledge that the classes have given them more confidence and helped awareness of what to do to prevent a fall. In addition, they have made modifications at home, see an eye doctor once a year, and review their medications. G) TOBACCO CESSATION - The hospital's once per week, four week Tobacco Cessation Program was expended to a six week program, which includes a behavioral health component. It is facilitated by a social worker specializing in addiction and a nurse practitioner from Behavior Health, as well as a community health nurse. The program is free-of-charge. The results for 2018 are: 1) 42 individuals registered; 26 attended, 2) 10 quit smoking, 3) Post-program - 13 individuals responded to follow up phone calls; 77% of those who did not quit wish to try again and recommended the program; 39% who did not quit cut down on their smoking.

Improving Access to Care

WOMEN'S HEALTH: NYU Winthrop's Women's Wellness Center in Hempstead addresses the health disparities of this select community by offering a wide variety of obstetrical and gynecological services. Free walk-in pregnancy testing is available Monday through Saturday. Services include on-site risk assessment, smoking cessation support, genetic counseling, nutritional counseling and healthy weight programs. High risk pregnancy patients may be seen by onsite maternal fetal specialist. Gestational diabetes education and monitoring are provided by the nurse practitioner. To meet the needs of the community, the site offered a Medicaid Enrollment Specialist for uninsured patients; Financial assistance and self-pay rates are available as well. The Center participates with NYS Medicaid and most Medicaid Managed Care HMO plans. The Financial Services Department meets with patients to facilitate Medicaid enrollment; this does not include those already enrolled in Medicaid. Additional services include: a) Psychosocial counseling, depression screening at each visit, post partum depression screening, bereavement counseling, education on domestic relationship abuse, assessment of living conditions, and cultural diversity; b) Breast feeding education performed throughout the pregnancy with focus on helping moms achieve optimal breastfeeding goals; c) Patients are counseled and educated on a variety of family planning options. Young moms are provided support by means of referral to home care and social work services coordinated through NYU Winthrop Hospital; d) GYN services include STD screening and education for prevention, education on many types of contraception options as well as vaccinations to prevent cervical cancer. The providers perform GYN procedures on-site, such as biopsy, colposcopy, and LEEP. Patients are provided referrals to NYU Winthrop specialists, e) For those in need of food, maternity and infant clothing, or infant necessities, WWW has a network of resources within the community that provide these items at no cost. WWW collaborated with the Nassau County Infant Car Seat program to provide education on the installation of car seats as well as adult and seat belt education. CHILDREN'S HEALTH SERVICES - A) The Hempstead High School Health Center (HHHC) is a school-based Health Center in a community with health disparities. Comprehensive preventive, episodic and confidential healthcare is provided. The center includes specific programs needed by the school's population, including: 1) Behavioral Health Initiatives that screen each patient at the center once a year for substance abuse and depression, utilizing the SBIRT Screening evidence based model (Screening, Brief Intervention, Referral to Treatment), and PHQ9 - a multipurpose instrument for screening, diagnosing, monitoring and measuring the severity of depression. Four staff members are also trained in NARCAN, 2 Youth Support Groups in English and Spanish that address common adolescent issues such as acculturation, adolescent adjustment, alternative lifestyles, teen pregnancy and prevention, parenting teens, and violence prevention, including gang violence, 3) an Asthma Management Program that monitors and tracks students with asthma. This effort is particularly significant in Hempstead, where asthma rates are nearly double those of adjacent neighborhoods. Both staff nurses in the HHHC are certified asthma educators. Collaboration with primary care physicians in the community facilitates treatment that meets current medical standards, trains high school nurses and staff about asthma, offers access to medications and assists with obtaining health insurance for uninsured students. There is a noticeable difference among students with asthma, in that fewer students are presenting at the clinic with difficulties, and 4) an Oral Health program that assesses dental needs and applies fluoride varnish. Additional Community-benefit Services at Winthrop

A) Blood Drives - During 2018, 937 pints of blood were donated at NYU Winthrop. B) Community Education - Approximately 2200 individuals attended free hospital-based lectures, addressing topics such as Heart Health, Diabetes, Stress Management, and more. Approximately 400 individuals attended free community-based wellness lectures that were requested by various organizations.Long Island is a large sports community. Approximately 150 young athletes, their parents and coaches attended lectures raising awareness about concussions, injury prevention and substance abuse. Stop The Bleed, a national program supported by the American College of Surgeons and Homeland Security, encourages everydaycitizens to become empowered to assist in bleeding emergencies until professional medical help arrives. Twenty three sessions were held for the community with a total of 509 attendees. C) Center for Family Dental Medicine Developed to target communities with health disparities, the Center offers specialized diagnostic and oral care from preventive measures which range from routine care and cleanings to more complex oral and maxillofacial surgery. During fiscal year 2018, the Center provided approximately 350 free screenings to participants at the Make a Kid Smile Event, as well as participated in several health fairs to provide information about the link between oral care and health. D) Center for Advanced Care of Chronic Conditions - NYU Winthrop offers a free service to the community to help individuals with a number of chronic conditions navigate the healthcare system. A nurse navigator customizes a plan of care, coordinates treatment and follow-up care, and promotes awareness of NYU Winthrop's various preventive and health screening programs. E) Health Fairs - During 2018, NYU Winthrop encountered approximately 1300 individuals at local community fairs, providing educational material about nutrition, blood pressure, cholesterol, nutrition, stroke and more. F) Health Screenings - Seventy individuals were screened at a free Stroke Risk Assessment, 466 men participated in free prostate cancer screening, and approximately 550 individuals received free blood pressure screening at various events. G) - Meals on Wheels - During 2018, NYU Winthrop provided 9,720 meals for homebound community residents. H) - Supporting Mental Health and Addressing Substance Abuse - During 2018, NYU Winthrop added 2 new social workers and 1 additional psychiatrist to help address the public's concerns with mental health. A Maternity/women's mental health program focusing on pre and post-partum depression, as well as other women's mental health issues, was started. An Alzheimer's Disease Center of Excellence affiliation clinic was started to meet the needs of this growing population. Referrals to Community Resources - NYU Winthrop has a solid referral base for community members in need of assistance for both mental health and substance abuse. The Department of Emergency Medicine, partnering with the Town of Hempstead, conducted a free Narcan (r) training for the community; 94 individuals were trained. All participants received free Narcan(r) kits. Approximately 175 free stroke support group sessions that are open to the public were held on topics such as cancer support, stroke, breastfeeding, bereavwment, Parkinson's Disease, Alzheimer's, and more. I) NYU Winthrop Welcome Center - The Welcome Center at the NYU Winthrop Wellness Pavilion in Garden City serves as a resource for written educational material, lectures and wellness programs, including New Parent Education and support groups. Also, Community residents calling the 1-866-WINTHROP number for information are directly linked to clinical departments as well as given referrals to community physicians and services as needed. J) NYU Winthrop Winnebago - NYU Winthrop's Winnebago is used to bring health education information and screenings to various locations throughout the community.
