Such payments made during calendar year 2012 are reported in Schedule J Part II Column B (iii)"Other Reportable Compensation" for the following individuals-Janeanne Lubin-Szafranski ($122,525), James Rude ($45,676), John Tarutis ($38,110), Christopher O'Connor ($740,875), Lawrence McManus ($570,105). Certain Saint Raphael Healthcare System, Inc. executives were eligible to participate in a supplemental non-qualified retirement plan through a date in calendar year 2010. During calendar year 2012 no provision was made for participation in such plan. Vested amounts are reported as a component of the value reported in Schedule J Part II Column B (iii) "Other Reportable Compensation" for the following individual- Janeanne Lubin-Szafranski ($12,579).
