Provide healthcare and social services to frail elders in the greater northshore communities by keeping them at home with dignity and respect.
The organization's mission is to provide health and social services to nursing home eligible clients in the greater lynn and northshore communities. Pace is a program of all-inclusive care for the elderly that enables eligible seniors to remain in their own communities rather than being placed in long-term care facilities. The organization's goal is to maximize the health and functioning levels of program enrollees to enable them to continue living in the community.

THE ORGANIZATION HAD AN INCREASE IN PROGRAM SERVICES ENROLLMENT.AS A PRIMARY CARE SITE OF THE COMMONWEALTH CARE ALLIANCE (CCA), THE ORGANIZATION PROVIDES SERVICES TO SENIOR CARE OPTIONS (SCO) ELIGIBLE SENIORS IN THE GREATER LYNN, NORTH SHORE AND CAPE ANN AREAS. THE ORGANIZATION MAINTAINS OPERATIONS OF THE ADULT DAY HEALTH FACILITY AND IMPLEMENTED A NEW PACE PROGRAM WITHIN THE NEVINS FACILITY. THE CLOSING OF THIS TRANSACTION OCCURRED ON OCTOBER 7, 2013, AND PRIOR TO THE CLOSING DATE, THE ORGANIZATION OFFERED EMPLOYMENT TO ALL CURRENT EMPLOYEES OF NEVINS ASSOCIATED WITH THE ADULT DAY HEALTH FACILITY. |
